Objective:To study the different ways the clinical curative effect of treatment of endometriosisand analyze the related risk factors for recurrence of endometriosis and relevant protectivefactors, so as to provide reference for our clinical, selecting reasonable treatment.Methods:Met inclusion and exclusion criteria by the example of surgical treatment of 180 patients with intrauterine membrane endometriosis, of which 67 patients with purelaparoscopic surgery EMS for Ⅰ group, 62 cases of EMS patients treated by traditionallaparotomy for Ⅱ group, 51 cases with laparoscopic surgery mifepristone in the treatmentof endometriosis patients for Ⅲ groups. Were retrospectively analyzed the clinical data,investigate the clinical efficacy of different treatment endometriosis and analysis of therelated factors of recurrence of endometriosis. Follow-up their current degree of pain relief,with and without pregnancy and whether relapse.Results:1、The laparoscopic operation group(118 cases) and open operation group(62 cases),operation time and the incidence of postoperative complications, no significant difference(P > 0.05), but laparoscopic operation group compared with open operation in lessbleeding, postoperative exhaust time and hospital time was shortened obviously(P < 0.05).2 、 The laparoscopic operation combined with mifepristone in the treatment ofpostoperative pain control effect is better than pure laparoscopic operation group,laparoscopic group than in open operation group(P < 0.05).3、Group I patients postoperative pregnancy rate(59.3%) and group II patients(56%),no significant difference(X2=0.249, P>0.05); group III patients postoperative pregnancyrate(30%) than in group I patients(59.3%), with statistical significance(X2=4.627,P<0.05).4、 Group I patients postoperative recurrence rate(17.9%) lower than those in groupII patients(29%), the difference was statistically significant(P<0.05); group III patientswith postoperative recurrence rate(11.8%) than in group I patients(17.9%), the differencewas statistically significant(P<0.05).5、The patients with endometriosis recurrence and patient age, lesion location, clinicalstaging, operation scope and other relevant factors.Stage III and IV patients relapse rate(38.7%) was higher than that of stage Ⅰ + Ⅱ(17.8%), deep invasive recurrence rate(42%)was higher than that of ovarian type(22.8%), conservative operation recurrence rate(33.9%) was higher than that of radical operation group 0, recurrence rate(19.6%) lowerthan untreated group relapse rate(37.2%), the differences were statistically significant(P<0.05).Conclusions:1 、 The operation therapy can effectively improve symptoms of endometriosis,improve the pregnancy rate, but the laparoscopic operation compared to the traditionalopen operation less bleeding, postoperative exhaust time, hospitalization time is short, andthe operation is done more thoroughly.2、The laparoscopic operation combined with mifepristone treatment helps maintaincurative effect, reduce the recurrence rate obviously, but hope for the pregnancy, notapplication of mifepristone treatment after operation.3、 The clinical stage high and deep infiltrating endometriosis were risk factors ofrecurrence, and age >45 years old, after application of mifepristone as a protective factor ofpostoperative recurrence.
