Objective:Depending on randomized controlled group and prospective experimental research methods, this study had a observation at the clinical effection of method combined herbs of smoothing live and activating blood for treating Rheumatoid Arthritis at active stage of damp-heat impeding syndrome. The study aim at having a result of the observation and evaluation of its recent clinical efficacy, safety, and the improvement of the quality of daily life in patients with RA at active stage. Thereby we can have a clear cognize that tells us the clinical features and advantages of the method for RA at active stage. As a result, the basement of evidence-based medicine to provide effective and reliable basis for this clinical treatments will be offered.Methods:Using experimental approach which took positive drug as randomized control group,67 cases with RA at active stage of damp-heat impeding syndrome met the inclusion criteria were selected,which were randomly divided into treatment group (34 cases) and control group (33 cases)by the random number table. The control group took only the decoction of Xuanbi and Sanmiao, while the treatment group was treated by the decoction of Xuanbi and Sanmiao combined with herbs smoothing live and activating blood. With a treatment cycles of 4 weeks, results(such as joint pain, joint tenderness, joint swelling, morning stiffness, joint activities, etc.)before and after 2 weeks,4 weeks medication and observation of clinical signs and symptoms of the two groups and laboratory parameters (such as ESR, CRP, blood routine test, urine routine test, liver function, kidney function) were recorded. Joint symptoms scale, DAS28 score, HAQ index, ACR response critetia were calculated before and after 2 weeks,4 weeks treatment as well. With comparative statistical analysis, the study tried to objectively reflect the traditional Chinese medicine treatment combined herbs of smoothing live and activating blood for treating Rheumatoid Arthritis at active stage of damp-heat impeding syndrome and the security.Results:1.In 2 weeks of treatment, ACR20 response critetia of the treatment group was 41.94%,much better then the 31.25% of the control group with significant difference(P<0.05).In 4 weeks, ACR20\50 response critetia of the treatment group was 70.97% and 35.48%,much better then the 59.38% and 28.13% of the control group with significant difference(P<0.05).2. In 2 weeks,4 weeks of treatment, joint pain, joint tenderness, joint morning stiffness of both groups were improved, while the improvements of treatment group were higher then those of control group with significant difference (P<0.05). Joint swelling and joint activities improvements of both groups were with no significant difference during 2w and 4w(P>0.05).3. In the disease index DAS28, HAQ index, laboratory parameters ESR、CRP improvements, when two weeks of treatment with adjuvant effect in group 4 weeks of treatment group were superior to traditional Chinese medicine, with a statistically significant difference(P<0.05).Conclusion:Method of clearing heat and eliminating damp combined herbs of smoothing live and activating blood can reduce ESR, CRP levels, disease activity and improve the clinical efficacy of joint pain, joint morning stiffness in patients with rheumatoid arthritis at active stage of damp-heat impeding syndrome, which was superior to using clearing heat and eliminating damp method alone. It showed no significant adverse reactions and side effects but high security. This treatment based on the theory " stagnation of liver causes Bi syndrome" is superior.
