ERUS With Water Enema In The Diagnosis Of Rectal Neoplasm

BackgroundTo investigate the accuracy of endorectal ultrasound(ERUS) with water enema on rectal neoplasm, and evaluate its clinical value. MethodThis paper collected 98 cases of rectal neoplasm patients. All of them were examined by ERUS without water enema, and then examined by ERUS with water enema. The diagnosing of tumor invasion(T staging) and local lymphatic node metastasis(N staging) was compared with result of operations research and histological stage of these patients. Accuracy of T staging sensitive and specificity of N staging were evaluated between ERUS with and without water enema. Results86 cases by ERUS with water enema were detected, and the accuracy was 88.0%(86/98); 56 cases by ERUS without water enema were detected, and the accuracy was 57.1%(56/98).There was statistical difference between these two kinds of methods(χ2 = 14.9, p <0.05).The accuracy between this two kinds of methods on the stage of PT1 and PT2 in the rectal cancer patients was statistical significance(p<0.05); there was no statistical significance(p> 0.05) on the stage of PT3 and PT4 in the rectal cancer patients.The accuracy, sensitivity and specificity of N staging by ERUS with water enema were 71.4%, 69.4%, and 74.1% respectively. The accuracy, sensitivity and specificity of N staging by ERUS without water enema were 65.1%, 63.9%, and 66.7% respectively. There was no statistical significance between these two kinds of methods(p> 0.05). ConclusionERUS with water enema has great value in the diagnosis of rectal neoplasm, especially for rectal cancer and inflammations.ERUS with water enema can accurately assess T stage of rectal cancer although N staging remains suboptimal. This is a useful and reliable method for preoperative T staging especially in diagnosing early stage rectal cancer.
