Background and purposeThe endometrial cancer is reproductive system disease in female,which occurred commonly in postmenopausal women and the incidence rate and high recurrence rate is high, proportion of gynecological tumor disease is high from 20% to 30% in women, The endometrial cance occurred in 50 ~ 60 years old in postmenopausal women, and perimenopausal women 25%. In recent years, incidence of endocrine and metabolic disease in women is increaseded with people’s living Standard improved and the change of life style and habits, which lead to endometrial carcinoma showing the trend of younger. the accurate judgment of patients with endometrial cancer is especially important for patients, radiotherapy, chemotherapy, the choice of operation mode and other treatment is selected through the pathological stage, myometrial invasion and lymph node metastasis, especially depth of myometrial In vasion influence operation range and postoperative greater in patient. At present, more diagnosis and assessment of endometrial cancer is applied including MRI, B ultrasound, CT and PET, while MRI is the fastest-growing, diagnostic accuracy, wuich has high diagnostic value on endometrial carcinoma before operation. VEGF-C as a marker of angiogenesis, which are closely relatedt to tumor invasion, infiltration and metastasis, promoting angiogenesis and vascular permeability.The purpose of this research is to study relationship among myometrial invasion of uterine and pathology, molecular biology characteristic, The malignant degree is speculated through the preoperative staging of endometrial carcinoma, and combined with the expression of VEGF-C of,tumor biological markers,wnich increase the scientific of clinical evaluation and prognosis, providing reference information for the treatment of tumor. Materials and Methods62 cases of endometrial cancer patients confirmed by pathology in January in Luohe Central Hospital from January 2011 to january 2011, retrospective analysis of Clinical data. They undergone MR examination with conventional sequence, dynamic contrast-enhanced imaging and DWI( b=0, 1000mm2/s)by 1.5T magnetic resonance scanner made in Shanghai General Electric(GE).10 cases of Uterine deformity andnormal endometrium patients as control group. Aceordingto FIGO2000 endometrial cancer surgery-pathologic staging, endometrial cancer was phased. All scans were performed on a ADW4.2 work station for image analysis and processing, two pathologists reviewed and observe with double blind method. Tumor tissue was stained with HE, and immunohistochemical SP method detected expression of VEGF-C in endometrial carcinoma. the data analysis and processing using was done by SPSS19.0 statistical software, the P<0.05 was considered statistieally signifieant. Results49 cases DEC-MRI imaging in 62 cases of endometrial cancer met the diagnostic pathological stage, diagnostic accuracy rate was 79%. Which Ia state of 27 cases of diagnosis in 31, the diagnosis accuracy rate is 87.1%.Ib state of 10 cases, diagnosis 7 cases, the diagnosis accuracy rate was 70%. II state of 12 cases, diagnosis 10 cases, the diagnosis accuracy rate was 83.3%. III state of 9 cases, diagnosis 5 cases, the diagnosis accuracy rate was 55.6%.56 cases DEC-MRI imaging in 62 cases of endometrial cancer met the diagnostic pathological stage, diagnostic accuracy rate was 90.3%. Which Ia state of 31 cases of diagnosis in 34, the diagnosis accuracy rate is 91.2%.Ib state of 11 cases, diagnosis 9 cases, the diagnosis accuracy rate was 81.8%. II state of 12 cases, diagnosis 11 cases, the diagnosis accuracy rate was 91.7%. III state of 5 cases, diagnosis 5 cases, the diagnosis accuracy rate was 100.0%.MRI examination of myometrial invasion cervical stromal invasion and lymph node metastasis has high accuracy in endometrial carcinoma,, respectively 83.9%, 96.8% and 91.9%. endometrioid adenocarcinoma was 59 cases in 62 cases of patients with endometrial cancer, of which three groups of signal strength, ADC values in the obturator internus were compared with each other,there was no significant difference in endometrial cancer(P>0.05), While signal intensity and ADC value were compared from well-differentiation, moderate-differentiation and poor-differentiation. well-differentiation and poor-differentiation, there was significant difference between moderate-differentiation and poor-differentiation in endometrial cancer(P>0.05). there was no significant difference between well-differentiation, moderatedifferentiation(P<0.05).Expression of VEGF-C was positively correlated with MRI staging of endometrial cancer. Conclusion 1, DCE-MRI combined with DWI imaging MRI has the advantages of multi-directional, multi angle imaging and higher resolution of soft tissue,which can clearly display the location, size and scope of endometrial carcinoma. 2, the expression of VEGF-C was positively correlated with MRI staging in endometrial carcinoma. 3, DCE-MRI combined with DWI imaging has judgment accuracy of preoperative staging of endometrial carcinoma, myometrial invasion and lymph node metastasis.
