Objective: To investigate whether partial aortic occlusion with the aortic balloon(Neuro Flo) can promote the rabbit cerebral ischemic model to establish the collateral circulation.Methods:1 Subselective transfemoral carotid digital subtraction angiograghy(DSA) was performed in 45 Japanese Rabbits.To observe the cerebral angiographic anatomy and variations of the circle of Willis. 2 Ligaturing the right carotid artery of all the rabbits to establish the cerebral ischemic model,after completing the DSA examination.One hour after the cerebral ischemic model were established successfully,the models in experimental group received the treatment of partial aortic occlusion for 45 minutes,the models in control group did not received any treatment.All the rabbits models were received the DSA examination at 30 mins,3h after the cerebral ischemic model were established.All rabbits models were received CT perfusion imaging(CTPI) before the models were established and after at 30 mins,3h,6h.Repeated measures design analysis of variance was used to compared the cerebral blood flow(CBF),cerebral blood volume(CBV),time to peak(TTP) between the experimental group and control group.Results:1 44 of the 45 rabbits had DSA examination successfully.The variations in the circle of Willis include asyammetry,hypoplasia etc.Only 40% circle of willis are complete without variations. 2 The cerebral ischemic models were established successfully in 40.Among them,20 are randomly selected into experimental group,20 were into control group.The left common carotid artery(CCA) angiography showed the anterior communicating artery was open in the experimental group.The left internal carotid artery(ICA) blood supply to the right internal carotid artery(ICA) by the anterior communicating artery.The right internal carotid artery and its branches development was developed well,and collateral circulation was builded.But in the same time,the anterior communicating artery was not open completely,there was no obvious collateral circulation in the control group. 3 The CTPI showed the control group hypoperfusion,ischemia area increasing gradually as the extension of time.while the experimental group was characterized that the low perfusion brain area was reducing gradually,after partial aortic occlusion.cerebral blood flow decreased gradually,while cerebral blood volume rised slightly at 3h,decreased at 6h. and time to peak extended in the control group.The mean cerebral blood flow in the experimental group increased after partial aortic occlusion,leading to low perfusion improved.while,cerebral blood volume increased and time to peak decreased.the difference between two groups was statistically significant(P < 0.05).Conclusions:The research confirmed effect of partial aortic occlusion(balloon expansion)to promote rabbit cerebral ischemic models to establish collateral circulation and its clinical significance.It provide new ideas and methods For further application in the clinical treatment of ischemic cerebrovascular disease.especially those patients that are not suitable for balloon expansion and stent implantation,or those patients with taboo of thrombolysis.
