Objective: To study the changes of coagulation function in patients with moderate,severe craniocerebral trauma and its influence on prognosis.Method:50 patients with moderate,severe craniocerebral trauma,accord- ing to degree of brain injury,they were divided into moderate group where there were 29 cases,severe group where there were 21 cases,the other 25 cases of healthypeople were included in healthy control group,the patients’ coagulation function and prognosis.Were compared among two groups and healthy control groups in blood.Result:1 The PT of patients in group 4 hours,12 hours,24 hours,48 hours,72hours were significantly higher than that in healthy control group(P<0.05),the PT of two group at 12 hours after injury was gradually increased,and after 24 hours decreased gradually(P<0.05),at different time points and that in severe group was longer than that in moderate group(P<0.05).PT of patients in two groups within at 12 hours after injury gradually increased,and at 24 hours after injury decreased gradually(P<0.05).2 The APTT at 4 hours, 12 hours, 24 hours after injury in moderate group were longer than that of healthy control group(P<0.05),at 48 hours,72 hours had no significant difference(P>0.05). the APTT at 4hours,12 hours,24 hours and 48 hours in severe group were significantly longer than that in healthy control group(P<0.05),at 72 hours had no significant difference compared with healthy control group(P>0.05).3 The PTT at 4 hours 12 hours after injury in two groups increased gradually(P<0.05),followed by 24 hours,48 hours,72 hours decreased gradually(P<0.05),the PTT at 72 hours after injury in moderate group had no significant difference with normal control group,the PTT in different time after injury in severe group were significantly higher than those in moderate group and healthy control group(P>0.05).4 The FDP within 24 hours of two groups gradually increased, at 48 hours,72 hours decreased gradually,at different time points the FDP were higher than those in healthy control group(P<0.05),and in severe group the FDP at 4 hours,12 hours, 24 hours,48 hours and 72 hours were significantly higher than that in moderate group(P<0.05).5 The D-dimer at different time points in Two groups were higher than that in healthy control group,at 4 hours,12 hours,24 hours,48 hours,72 hours after injury decreased gradually(P<0.05),the D-dimer in severe group at different time points were higher than that in moderate group(P<0.05).6 The mortality and bad prognosis rate in severe group were significantly higher than that in moderate group,prognostic and good rate was lower in moderate group(P<0.05).Conclusion: patients with moderate or severe craniocerebral injury after injury may occur early coagulation dysfunction,the degree of coagulation dysfunction is higher,influence on prognosis is more serious.
