Objective To investigate the diagnostic value of TI-RADS combined with CEUS in differentiating between thyroid malignant masses and benign masses.Methods 91 patients with 127 nodules in the study were examined by ultrasound and CEUS. The nodules were graded by TI-RADS based on 2D-US features of benign and malignant, and proved by pathologic findings. By taking pathologic findings as golden standard to evaluate TI-RADS, CEUS and their combination diagnostic value of thyroid cancer. Result Early hypoenhancement, heterogeneous enhancement and no ring enhancement surrounding are CEUS enhancement pattern for thyroid cancer. Early hypoenhancement has the highest diagnosis efficiency, which sensitivity, specificity and accuracy were 73.08%,92.07% and 88.19% respectively.The sensitivity, specificity and accuracy of TI-RADS were 46.15%,63.36% and 61.41% respectively. The combination of the two methods could bring higher sensitivity of 80.76%, specificity of 96.04% and accuracy of 92.91% than single method. Three methods of diagnosis accuracy difference was statistically significant (X2=47.31, p=0.000<0.05). Conclusion TI-RADS could give a preliminary and specification malignant risk judgment to thyroid lesions, the use of CEUS can further improve the capability of differential diagnosis, the combination of TI-RADS and CEUS could increase the preoperative diagnosis capability of thyroid nodules.
