The Clinical Observation Of Modified Sangpi Tang In The Treatment Of AECOPD Phlegm Heat In Lung And The Effect On IL-8 And Other Inflammatory Mediators

Objective: To observe the clinical efficacy of modified sangpi tang on acute exacerbation of chronic obstructive pulmonary disease(AECOPD) phlegm heat in lung and its influence on serum inflammatory mediators(IL-8,TNF-?,hs-CRP).Methods: Will meet the inclusion criteria of AECOPD due to phlegm heat obstructing lung, 66 cases were randomly divided into treatment group 33 cases and control group 33 cases, 6 cases fall off in the observation process,the remaining30 cases in the treatment group,30 cases in the control group.The control group received oxygen therapy, anti infection, diastolic bronchial, expectorant and other conventional treatment of western medicine, the treatment group received the basis of western medicine plus modified sangpi tang, daily a agent, 14 days later, statistical analysis two group patient’s main symptoms integra, syndrome effect, pulmonary function, routine blood test, use of antibacterial drug and serum inflammatory mediators(IL-8, TNF-?,hs-CRP) changes.Results:(1)The main symptoms integra:Two groups patients’ symptoms(cough, sputum volume, character of sputum, dyspnea,wheeze, fever)scores were significantly improved than before treatment, the difference was highly significant(P<0.01), and the treatment group in improving the patient cough, sputum volume, sputum character, dyspnea,wheeze integral was better than the control group, the difference was significant(P<0.05).The two groups patients’ symptom total integral was significantly reduced than before treatment, the difference was highly significant(P<0.01), and the treatment group’s total score decreased more obviously, the difference was significant(P<0.05).(2)The syndrome effect:The total effective rate and significant efficiency of the treatment group were 96.7% and 83.3%, the total effective rate and significant efficiency of the control group were 93.3% and 56.7%, the syndrome effect of the treatment group was better than control group, the difference was significant(P<0.05).(3)The pulmonary function: After treatment the indexes of pulmonary function of two groups(FEV1, FEV1%, FEV1/FVC) were obviously increased, the difference was highly significant(P<0.01), but the two groups after treatment was no significant(P>0.05).(4)The routine blood test: Routine blood test(WBC, N%) of the two groups two groups after treatment were significantly lower than before treatment,the difference was highly significant(P<0 01),the treatment group after treatment compared with the control group decreased more significantly(P<0.05).(5)The use of antibacterial drugs: Treatment group at time antibacterial drugs use time was significant shorter than that of control group, the difference was significant(P<0.05).(6)The inflammatory mediators in serum: The level of inflammatory mediators(IL-8,TNF-?,hs-CRP) in the two groups after treatment were significantly lower than before treatment,the difference was highly significant(P<0.01), the treatment group after treatment compared with the control group decreased more significantly, the difference was significant(P<0.05).Conclusion: Modified sangpi tang combined with western medicine treatment of AECOPD curative effect is more significant, and can quickly improve the body symptoms of patients with AECOPD,improve syndrome effect,reduce the blood routine index,shorten the time of the use of antibacterial drugs and can significantly reduce serum inflammatory mediators(IL-8, TNF-?,hs-CRP)level.
