Objective:Under the protection of inferior vena cava filter, using catheter thrombolysis and dorsalis pedis vein thrombolysis this two different thrombolytic therapy in the treatment of lower limb deep vein thrombosis formation(DVT), comparing the two kinds of thrombolytic therapy early clinical curative effect.Methods: general surgery in our hospital in June 2012-2014 in June of DVT patients, consistent with no previous history of DVT, deep vein thrombosis occurred in a week time, and because of the condition of patients thrombolytic treatment and consent to treatment 86 patients for the study. Randomly divided into dorsal venous thrombolysis group(N = 36) and catheter thrombolysis group(N = 48). Evaluate the efficacy of early improvement through the vein patency and clinical symptoms.Results: 86 patients, two patients were implanted in the inferior vena cava filter fails it does not start thrombolytic therapy, 84 patients successfully implanted, when then 48 cases of patients with thrombolytic therapy catheter placement, 43 patients suffering from 3 days limb swelling subsided significantly, the patient returned to normal 37 cases a week; two weeks when the limb venous patency rate was 77.1%, while the use of dorsal venous thrombolytic therapy in 36 patients, 16 patients three days limb swelling subsided significantly the patient returned to normal a week 9 cases; two weeks when the limb venous patency rate of 25%, intravenous thrombolysis in patients with catheter patency rate was higher than the dorsal venous thrombolysis group, two groups of patients before surgery circumference difference no comparison statistically significant(P> 0.05), two groups of patients after the first three days, one week, two weeks when the limb with the healthy limb circumference difference, significantly lower than the preoperative(P values were 0.003,0.001,0.002), and the catheter thrombolysis group and the healthy limb limb circumference difference was significantly lower than after intravenous thrombolysis dorsal group, the difference was statistically significant compared two groups(P <0.01). In terms of venous patency, 77% catheter thrombolysis group is also much higher than the 25% dorsal venous thrombolysis group.Conclusion: Under the protection of inferior vena cava filter, the application of dorsalis pedis vein thrombolysis in the treatment of DVT can obtain certain curative effect, but the application of indwelling catheter thrombolysis can be more rapid improvement in lower extremity venous reflux disorder, vein patency rate significantly increased, compared with foot dorsal vein thrombolytic effect is more significant.
