Objective Questionnaire survey were conducted for target population,analysis of the physical measurement, serum index and body composition,study the relationship of the serum levels changes of Lipopolysaccharide(LPS),interleukin-18(IL-18),interleukin-1β(IL-1β) with nonalcoholic fatty liver disease(NAFLD),fresh feces were collected for intestinal flora analysis from the research object,to explore the relationship between patients’ intestinal flora changes who with nonalcoholic fatty liver disease and dietary factors,LPS, IL-1β, IL-18.Method Choose health check-up crowd who have a medical examination in a hospital physical examination center in 2013,questionnaire survey was conducted for all selected populations(the history of alcohol, drugs and liver toxicity substance exposure history and family history of diabetes and coronary heart disease,etc was included.), physical measurements(height,body mass index,and waist circumference,body composition analysis(visceral fat area) and liver ultrasound;The physical examination reports of selected population were collected.The general situation of individuals and different kinds of food and the intake amount what they eated was investigated by using self-designed health questionnaire and food frequency questionnaire, respectively.The research object was divided into NAFLD group and the control group,analysed of general situation of two groups and compared the human body composition index and biochemical index;serum levels of lipopolysaccharide,interleukin 18 and interleukin 1βwereexamined by enzyme-linked immunosorbentassay.At the same time handled stool specimens,extracted bacterial DNA to amplification real-time fluorescent quantitative PCR and analysed the differences of E.Coli and Lactic acid bacteria expression between the two groups.Result 1.BMI,VFA,PBF,WHR of NAFLD group is significantly higher than the control group(P<0.01),and the WBC,FBG,VLDL,ALT,UA of NAFLD group were significantly higher than that of control group(P<0.01),TG,NEUT,LYMP was also significantly higher than the control group(P<0.05),with statistical significance;there were no statistically significant differences in AST,TC,Cr of the two groups. 2.The concentration of serum LPS in NAFLD group and the control group(ng/ml) was 172.64± 67.21 and 153.51 ±83.88(P = 0.302,P >0.05), no difference between the two groups;The concentration of serum IL-18,IL-1β in the NAFLD group and the control group(pg/ml) were 121.01±39.61,98.08±40.10,23.78±9.40,19.06±9.42,the NAFLD group were significantly higher than the control group,the difference was statistically significant(t = 2.550, 2.307, P = 0.013, 0.024). 3.The daily intake of total energy,fat,and protein for per person among the investigated subjects with NAFLD was higher than that of the normal control(NC) group,there was no significant difference in carbohydrates, dietary fiber. 4.There was no obvious difference between NAFLD group and normal control group of E. Coli(copy number/per gram wet feces),the copy number was4.62 ±1.20 and 4.59±1.67,respectively.The lactic acid bacteria decreased(3.45±1.45) significantly in NAFLD group, it was lower than the normal control group(4.20±1.41,P < 0.05). 5.LPS in NAFLD group are positively associated with BMI(r=0.349,P<0.05);IL-18 in NAFLD group are positively associated with BMI,WHR,TG(r = 0.361, 0.504,0.372, P < 0.05), IL-1β are positively associated with BMI,WHR,TG(r = 0.345, 0.343,0.465, P< 0.05).Lactobacillus group of NAFLD group are negtively trend associated with IL-18 、 IL-1β(r=-0.086 、-0.163, P=0.611 、 0.341),and no significant statistical significance.Conclusions 1.Serum levels of IL-18 and IL-1β in NAFLD group are higher than those in control group, IL-18 and IL-1β participating in Chronic inflammation may be one of the causes of NAFLD. 2.The intake of basic nutrients among the investigated subjects is adequate to satisfy the demand of human body, but the diet structure is unreasonable and some nutrients are either excessive or deficient.The correlation between the human body composition analysis and dietary factors is very large,the intake of high energy,high fat,high protein are the risk factors of NAFLD. 3.Intestinal helpful bacteria decreased significantly in NAFLD patients, it was lower than the normal control group,this suggests structural imbalance of intestinal flora in patients with NAFLD. 4.Structural imbalance of intestinal flora may be one of the reasons for the occurrence and development of inflammatory response in NAFLD patients.
