ObjectiveTo observe the effect of serum containing Jiegufang on proliferation of osteoclast, osteoblast and chondrocyte in cultured callus, and Jiegufang’s clinical efficacy on the avulsion fracture of the proximal fifth metatarsal. To explore the mechanism of Jiegufang decoction in promoting fracture healing.MethodsThe animal experiment:After Making the fracture Model of Newzealand rabbits, the callus was caught and cultured in vitro, Hatched with serum containing Jiegufang for 72 hours. Observing and recording the quantity of osteoclast, osteoblast and chondrocyte respectively at the time of 10d, 17d,24d,31d after operation. Statistical analysis were conducted on the data.The clinical research:Patients with a proximal fifth metatarsal avulsion fracture were selected in 2014.03-2014.12 (in Guangzhou University of Traditionnal Chinese Medicine Affiliated Hospital of orthopedics and traumatology). Patients from March to July were devided to treatment group (treating with jiegufang), and patients from August to September were devided to control group (no jiegufang). Both groups were treated with limb plaster fixation and elevation in the early time. And bearing gradually for 3 weeks. Observing and recording the degree of pain, swelling, tenderness, functional activities and other symptoms and signs, respectively before treatment, after treatment for 2 weeks, after treatment for 4 weeks. The results were statistically analyzed.ResultsThe animal experiment:Osteoclast number difference between two groups was not statistically significant respectively at 10’s day,17’s day,24’ s day,31’s day after operation(P>0.05);The number of osteoblast and chondrocyte in cultured group were statistically higher than that in control group (P<0.05)The clinical research:41 patients were followed up at last. The general information and injury severity between two groups had no significant difference(P>0.05). In the aspect of reliving foot swelling and tenderness, the Jiegufang group had a significantly better effect than the merely plaster slab group at 2 weeks after treatment(P<0.05). But no significant difference at 4 weeks after treatment(P>0.05).In the aspect of pain relief and functional recovery, the Jiegufang group got a significantly better score than the plaster slab group both at 2 and 4 weeks after treatment(P<0.05).Modified AOFAS Midfoot score in the Jiegufang group was significantly higher than that of plaster slab group at 4 weeks after treatment(P<0.05).ConclusionSerum containing Jiegufang can promote the proliferation of osteoclast, osteoblast and chondrocyte in the callus cultured in vitro. This may be one way how Jiegufang decoction promote fracture healing. It can relieve symptoms and signs occuring at the early and mid stage of the proximal fifth metatarsal avulsion fracture effectively, and accelerate functional recovery.
