ObjectiveAutosomal dominant polycystic kidney disease(ADPKD) is a common hereditary disease, caused by mutations of either PKD1 or PKD2. End stage renal failure (ESRD) is the most dreaded complication of ADPKD. To investigate the expression of the transforming growth factor-β1(TGF-β1) and a vascular endothelial growth factor (VEGF) in ADPKD.MethodsAccording to the preoperative glomerular filtration rate (GFR) of abnormal kidney, 72 ADPKD patients were divided into two groups:high GFR group (group A(GFR≥25ml/min), n=30), and low GFR group (group B(GFR<25ml/min), n= 42).Another 20 patients treated with radical nephrectomy were chosen as controls(the control group). Kidney tissues from patients in groups A and B, and tumor-adjacent normal tissues from the controls were collected. Expressions of TGF-β1 and VEGF of the three groups were detected with immunohistochemical method.Results1. TGF-β1 was positively expressed in ADPKD. No significant expression was found in the control group. The positive expression rate of TGF-β1 was statistically significant (χ2=7.133, P<0.05).2. VEGF was positively expressed in ADPKD. No significant expression was found in the control group. The positive expression rate of VEGF was statistically significant (χ2=5.247, P<0.05).3. The expression of the TGF-β1 in group B(low GFR group) was significantly higher than group A(high GFR group), and the positive expression rate in different group was statistically significant (χ2=6.583,P<0.05).4. The expression of the VEGF in group A(high GFR group) was significantly higher than group B(low GFR group), and the positive expression rate in different group was statistically significant (χ2=16.205, P<0.05).5. The expressions of TGF-β1 and VEGF were negatively correlated (r=-0.22).Conclusion1. The positive expression of TGF-β1 significantly increase with the decrease of GFR, and the positive expression of VEGF significantly reduce with the decrease of GFR. They simultaneously promote the progression of ADPKD, and suggest may play an important role in the pathogenesis of cyst formation and interstitial fibrosis in ADPKD.2. According to the expressions of TGF-β1 and VEGF were negatively correlated, suggest that the interstitial fibrosis in ADPKD significantly reduce with the regeneration of blood vessels.3. Because the TGF-β1 is widely distributed in the body and the specific biological characteristics is not yet clear, blocking the expression of TGF-β1 in the body may leads to some unexpected consequences. But by giving inhibit VEGF drugs for early intervention ADPKD and supplementary VEGF drugs to middle-late ADPKD intervention, and at the same time, through the detection of TGF-β1 concentration judge to the degree of the ADPKD. Whether the above method can become the feasible way which is reduce or delay the progress of ADPKD kidney tissue fibrosis.
