BackgroundsDiabetes is a clinical syndrome caused by lack of insulin secretion or insulin utilization disorder. Due to the long course of such disease, impairment on organs will be occurred with the progression of diabetes. In recent years, the improvement of living condition combined with the change of disease spectrum contributed to the ever increasing rate of diabetes, which has imposed a serious threat to the public’s health. Following the progression of the disease, it tends to impair the normal function of insulin cells and their disability to secrete, leading to the lack of insulin in organs. As a result, the level of blood glucose will rise, and majority of patients will have to resort to the alternative method which will use insulin from external sources rather than from human bodies.Insulin injection pen in a combination of insulin and syringe. It is portable, easy to operate and causes less pain, hence enjoys high popularity in patients suffering from diabetes. However, if it is operated in an incorrect way, the amount of insulin needed to inject will be not accurate and wrong injection will occur, which will significantly infect the rate of the patients whose blood glucose reach to the standard and their blind dependence on treatment.The research is based on the cases involved patients with diabetes II currently treated in the First Affiliated Hospital of Bengbu Medical College, An Hui, who use insulin syringes themselves. In addition, the research is conducted from two perspectives:current situation concerning the standard use of insulin syringes and the relevant influences.ObjectiveThe research aims at understanding the level of standardization related to the use of insulin syringes in patients and analyzes its influences factors, providing guidance for patients to operate insulin syringe in a more effective and safer manner. In addition, the research takes its objective helping those suffering from diabetes II control their blood glucose effectively and reduce or delay the occurrence or development of complications, therefore improving the living quality of patients with diabetes.MethodsBy means of Self-made Questionnaire, EQ-5D scale and insulin interviews Kit, the research is conducted in the way of questionnaire including the general socio demographic characteristics, economic conditions, payment methods of medical treatment, knowledge on insulin and on diabetes, and the situation on the standard use of insulin syringes. The survey is on the basis of the whole samples which involve 450 young and middle-aged patients with diabetes II treated in the department of secretion of the First Affiliated Hospital of Bengbu Medical College, An Hui.After data collection, analysis on factors influencing the standard use of insulin syringes was conducted with Logistic regression model, Logistic regression analysis showed:education, residence status in patients with type 2 diabetes, insulin cognitive status, usage charges insulin injection pen and whether to accept diabetes education is affecting their insulin pens use a major factor in the level of standardization.ResultsThe research reveals a low level of standard concerning the use of insulin syringes. Furthermore, if the disposable replacement of special needles for insulin syringes is taken into consideration when it comes to the standardization, only 7 in 450 cases could reach the standard with the rate of standardization being1.6% otherwise the rate will be higher arriving at 40%, with 180 patients using the insulin syringes in a standard manner. Analysis based on single factor (单因素分析)suggest the following factors have, to various degrees, effects on the standard use of patient with diabetes Ⅱ:the age of patients with diabetes (x2=73.717, P<0.01); living conditions(living with families or not) (x2= 16.725, P<0.01); education level (x2= 6.593, P<0.01); payment methods for medical care (x2= 32.568, P<0.01); income (x2= 16.087, P=0.001); years of using of insulin syringes (x2= 7.619, P=0.022); education on diabetes (x2= 37.855, P<0.01); regular check on blood glucose (x2=19.703, P<0.01) complication (x2=6.897, P=0.010); fees on insulin syringes (x2=6.507, P=0.039) knowledge on insulin (x2= 18.941, P<0.01)ConclusionsIn order to standardize the use of insulin syringes in patients with diabetes Ⅱ, the governments and relevant medical departments are supposed to put into practice and perfect systems in relation to the education and management of patients to reduce or delay the occurrence of related complications. Resources should be made full use of and multiple modes of education on public health should be promoted so that patients can have a better understanding of the standard use of insulin syringes. In addition, financial aid should be provided for patients using insulin syringes, and in particular, medical insurance should cover needles for insulin syringes so as to reduce patients’ financial burden.
