| ObjectiveWith the development of modern medicine and immunoglobulin detection technology research and extensive application,immunoglobulin detection technology application is more and more important in kidney disease, by detecting kidney patients serum immunoglobulin(Ig G,Ig M and Ig A) level, can be more accurately explore the pathological and clinical treatment of kidney disease to provide important reference basis, but also system to the kidney disease diagnosis, treatment and prognosis assessment has important guiding significance. MethodologyThis topic through clinical diagnose, kidney disease, diagnosis standard of hospitalized patients as the research object, the selection of laiwu city people’s hospital urology department from 2012 to 2013 by 80 patients with renal biopsy pathology diagnosis clear. This, according to different clinical diagnosis of 80 patients were divided into groups, Ig A nephropathy(Ig AN)group with lupus nephritis(LN) group, and kidney disease is minimal change disease(MCD) group, the other randomly selected 40 cases of medical healthy subjects as control group were compared. To fasting venous blood of all the observer, using, Hitachi high-tech companies with Hitachi 7600-020 type automatic biochemical analyzer by using the method of immune transmission turbidimetric serum immunoglobulin Ig G, Ig A, Ig M levels were determined. Normal reference value:(Ig G: 8.00 ~ 16.00 g/L; Ig A: 0.70 ~ 3.30 g/L; Ig M: 0.5 ~ 2.5 g/L). Using the statistical methods, to the data statistics and analysis, from the lab by detecting serum immunoglobulin levels in patients with different kidney disease, so as to further explore kidney patients serum immunoglobulin clinical indicators and the relevance of kidney system disease. Results1、In the kidney disease group and health control group, patients with Ig AN damage the highest(38.8±13.8) years old. The second is the MCD group, patient age was(34.3±15.7). LN patients age was(32.4±12.5), lower than other groups. LN patients mainly women, accounting for 81.2%. Ig AN patients age is broad, visible at any age, but mainly in young adults and children are common, the peak of the disease is between 20 to 40 years old.2、Patients with Ig AN and LN group is significantly higher than the control group, serum levels of Ig G, Ig A, statistically significant difference(P < 0.05); Ig AN groups with the rest of the control group, the serum levels of Ig A is significantly higher than other kidney disease group and the control group, the difference was statistically significant(P < 0.01); LN patients serum Ig G levels are significantly higher than other kidney disease group and the control group, the difference was statistically significant(P < 0.01), Ig M level compared with the other kidney disease patients change is small, there was no statistically significant difference(P > 0.05); MCD patients serum Ig G levels lower than other kidney disease group and the control group, the difference is statistically significant(P < 0.05). Conclusion1、Ig AN immune complex glomerular disease, Ig A level increases in serum, showed that humoral immunity and cellular immunity plays an important role in the pathogenesis of Ig AN.2、LN itself is a kind of immune complex disease, in most cases Ig G will produce the phenomenon of increased, initial activity in patients with LN Ig G levels rise is one of the characteristics with lupus nephritis.3、MCD patients serum Ig G levels compared with other groups, the main reason is that the patients of MCD is low serum albumin leels of nephrotic syndrome, which is one of the main reasons for MCD easy intercurrent infection.4、Immunoglobulin detection for the diagnosis, treatment and prognosis of kidney disease assessment has important guiding significance. But can’t rely on one or several changes in the level of Ig for the treatment of kidney disease, should refer to the influence factors of more, also want to combine more clinical and renal pathological observation indexes scientific comprehensive judgment. |
