ObjectiveWith the economic development and lifestyle change, environmental pollution, microbial infection, an aging population, activity decrease and obesity increases, the incidence of colorectal cancer incidence is high. The 5-year survival rate basic maintain at 60% to 70%,Neutrophil count, neutrophil- to lymphocyteratio, NLR, has been linked to a variety of closely related to the prognosis of malignant tumors.。 This study oncology clinical data from dongying city people’s hospital in February 2004 to February 2004 in our hospital line cases of colorectal cancer adjuvant chemotherapy, chemotherapy treatment in patients with colorectal cancer before neutrophil count, neutrophil and lymphocyte ratio and clinical pathologic factors and the relationship between the prognosis.Methodscollected in February 2004 to February 2009 in dongying city people’s hospital treated the needs of the data line data of 255 cases of adjuvant chemotherapy, chemotherapy cases 148 cases of male, female 107 cases,< 60 years old in 144 cases,111 cases,60 or higher average age 61.43, patients admitted to hospital after each cycle of chemotherapy before collecting peripheral venous blood on an empty stomach, neutrophil count, NLR testing, assessment before chemotherapy KPS score, adenoma history information, and statistics of the patient’s age, location, tumor size, infiltration depth, lymph node metastasis number, stage, pathological differentiation, cancer patients with stage 1 and stage of Ⅲ, Ⅳ line 8to 12 cycles of chemotherapy, followed up for 5 years, calculate PFS, OS observe the correlation of neutrophil count for colorectal cancer prognosis.2, establishment of follow-up for all patients 1 year,3 years,5-year disease-free surial rate, according to former chemotherapy peripheral venous blood NLR high value points NLR group (NLR 3 or higher) and less NLR (NLR< 3) in the two groups, analyzing the relationship between the two groups of NLR and disease-free survival time, and then rank and inspection of the NLR value analysis of various factors, to discuss the relationship between pre-chemotherapy NLR and various factors. Suggest was statistically significant (p< 0.05).Results1, chemotherapy before neutrophil count the median value of 4.85* 10 ^ 9/1. Male, Ⅲ period and a history of adenoma patients before chemotherapy neutrophil count median than women, Ⅳ period and no history of adenomas (P< 0.05); KPS score, tumor location, age, and different pathological types in patients with neutrophil count leels were no statistically significant difference.2, the late before chemotherapy increased neutrophil count no rise and OS a median of 18.5 and 13.7 months, respectively, was statistically significant (P< 0.05). Neutrophil counts increased with increased median PFS were 5.3 months and 8.7 months significantly shortened (P< 0.05). Single factor analysis:neutrophil count is greater than the critical value (HR= 1.66,95% CI:1.18~2.31, P< 0.05) and KPS score of 70-90 (HR= 1.88,95% CI:1.33 2.68, P< 0.05) is poor prognosis factors. COX multiple factors analysis, the results showed that neutrophils and KPS score of 70-90 points is still poor prognosis factors.3, Ⅲ colorectal cancer chemotherapy before NLR mean was 3.00 ± 2.31, Ⅳ period 3.17 ±2.18, the above two groups before chemotherapy NLR difference was statistically significant (p= 0.0441), with the later TNM staging, the greater the NLR before chemotherapy.High NLR group of patients 1 year,3 years,5-year disease-free surial rate was 68%, 53%,34%; Low NLR group was 80%,73%,67%. Differences between groups was statistically significant (p< 0.05). COX regression tip:before chemotherapy NLR, TNM staging is the independent factors influencing the prognosis of colorectal cancer chemotherapy.Conclusion1, neutrophils level before chemotherapy can judge the late first-line chemotherapy of colorectal cancer prognosis.2, colorectal cancer patients with chemotherapy before NLR is associated with TNM staging.3, chemotherapy before NLR= 3.0 as judging the prognosis of patients with colorectal cancer cutoff value, and the prognosis of patients with NLR of 3 or more than the NLR< 3 difference, colorectal cancer patients with chemotherapy before neutrophils and lymphocytes ratio (NLR) is higher, the worse prognosis.4, NLR before chemotherapy can be used as the judgment of the prognosis of patients with colorectal cancer.
