Objective:To estimate the efficiency and toxic reaction of fluorouracil derivatives to advanced gastric cancer after the induction chemotherapy. Methods:Retrospective analysis was performed in four hundred and thirty-two patients with advanced gastric cancers treated in our hospital form January 2009 to Julyjy 2014, and all patients achieved induction chemotherapy for chemotherapy used taxane and platinum drugs or fluorouracil. They achieved disease control, complete response(CR), partial remission (PR) or stable disease (SD) after the induction chemotherapy. One hundred and twenty-six patients were given Fluorouracil derivatives maintenance treatment. With a dose of capecitabine 1000-1250mg/m2,2 times/day, continually two weeks, there weeks for a period. With a dose of S-1,80mg/m2, two times, continually two weeks, there weeks for a period. Treatment follows disease progression or progression of last follow-up time. They were achieved two cycles of treatment at least. Another group is anothers patients without the maintenance treatment. The purpose was the median free survival rate and one and two-year survival rates. The capecitabine and S-1 adverse reactions were observed. To explore the relationship between the clinicopathological features and progonisis of these patients. Results:The median disease free survival time was 7.80 months for the maintenance treatment group and 5.42 months for control group (p<0.05); The median overall survival time was 12.11 months for the maintenance treatment group and 10.60 months for control group(P>0.05).the 1-year surviva Irate respectively was 38.1% and 23.3% (p<0.05);2-year survival rate respectively was 9.5% and 4.8%, (P>0.05);in the investigation, the complications include the hand-foot syndrome in twenty-one cases (16.7%), the arrest of bone marrow in fifty-six cases (44.4%), and the gastrointestinal reaction in forty-five cases (35.7%), the reaction which is mostly Ⅰ or Ⅱ degree could be tolerated well. Conclusion:Fluorouracil derivatives monotherapy maintenance treatment may prolong the median progression-free survival and one-year survivas rate, in addition, the good treatment compliance and the tolerated gastrointestinal reaction in patients are observed. Liver metastases, the number of metastatic lesions hepatic metastases and the number of cycles of chemotherapy were a significant independent prognostic factor for PFS and OS.
