Clinical Study Of The Surgical Approaches For Pediatric Intracranial Arachnoid Cysts

Objectivet:To investigate the different methods and clinical effects of operation strategy of pediatric intracranial arachnoid cysts.Methods:data of 32 patients with pediatric intracranial arachnoid cysts,admitted to our hospital from January 2011 to July 2013;all patients were examined by CT and MRI;Randomly divided into treatment and control groups,the treatment group were grouped two cases according to different operative techniques,fistulation was chosen in 17 patients(including endoscopic fistulation in 14 and microscopic fistulation in 3)and cyst-peritoneal shunt was chosen in 9;Without operation in 6 cases was control group.All cases followed up from 3 months to 18 months.The improvement in postoperation singns and symptoms of pations,the cysts volume changes in iconography and postoperation complications were retrospectively analyzed.Results:The contrastive difference in the cysts volume changes before and after the operation of the two kinds of surgical methods has statistical significance P>0.05). hatever the surgical method is,it can be able to make cysts reduse.The contrastive differense of the eventual reduction degree of the cysts has statistical significance P<0.05).Cyst-peritoneal shunt precede than Fistulation.Significant difference was noted in the surgical efficiency and the rate of recent complication between cyst-peritoneal shunt and fistulation(P>0.05).Both method can improve symptoms of children,occur certain degree of certain complications after all method.The rate of long-term complications in patients after cyst-peritoneal shunt insertion was significantly higher than that in patients after fistulation(P<0.05).Conclusion:Fistulation as a minimally invasive approach,is more safe, convenient,with less complications,Fistulation might be the preferred method for pediatric intracranial arachnoid cysts.
