BackgroundCerebral cysticercosis is a common infectious disease of the central nervous system, now the number of infections have increased year by year in our china,it is will give a threat to human health. Clinical manifestation complex symptoms have different weight about brain cysticercosis,, its morbidity is high, the prognosis is poor, so early detection, early treatment, clear pathological staging can maximum reduce the hazard is the key of the prevention and control. MRS technology can research group of compounds metabolism without trauma, in clinical application is very extensive, but the 1H-MRS value to the diagnosis of parenchymal type cerebral cysticercosis is very little research reported. ObjectiveDiscussion of parenchymal neurocysticercosis 1H-MRS characteristics, analysis of 1H-MRS specific performance about vesicles live insects and edema lesions; According to parenchymal neurocysticercosis 1H-MRS reasonable objective evaluation of the pathological staging of the disease; providing clinical diagnosis and differential diagnosis of the disease according to parenchymal neurocysticercosis 1H-MRS the performance characteristics. Materials and MethodsCollected in September 2013- December 2014 clinically diagnosed with brain parenchymal cysticercosis 54 cases(30 cases of anti-cysticercosis treatment is effective,24 cases of immune response positive enzyme-linked), 28 cases of male, 26 casesf of emale.The maximum age of 65 years old, the minimum age of 20 years old, the average age was 44.9 years old. 54 patients using a GE 1.5T-HDe conventional superconducting MRI axial T1WI-FLAIR, T2 WI, T2WI-FLAIR sequence scanning, according to the 22 cases were included in the standard of living insects vesicle lesions, 26 cases of degenerative lesions edema were included in the single voxel proton magnetic resonance spectroscopy(1H-MRS) imaging scan range, each patient to choose a lesion TE set point 144 ms single voxel resolution proton magnetic resonance spectroscopic imaging.After the scan is complete, the observed metabolite concentrations in different areas of interest, determination of live insects all vesicle lesions, degeneration edema lesions and normal brain tissue contralateral mirror area of interest, NAA,Cho, Cr metabolite concentrations,, and calculate the relative ratio of metabolites(Cho / NAA, NAA / Cr, Cho / Cr),observing whether there Gln, Lac, Lip and Ace and other substances.Using SPSS16.0 for data processing, data obtained with mean ± standard deviation(X± s) expressed interest in the lesion area vesicles live insects, degenerative lesions in each area of interest edema and contralateral normal brain image area of interest metabolite ratios using the t test analysis, each metabolite ratios vesicles live insects degenerative lesions in the region of interest and the interest area of edema lesions were compared using ANOVA, when P <0.05 was considered significant difference, it was statistically significant. a value is 0.05. Results 1 22 cases of live insects is slightly vesicles of the 1H-MRS show reduced NAA, Cho significantly increased, Cr remained unchanged, there has been one case of lesions Lip, six cases of lesions appear Ace, seven cases of lesions appear Gln, Lac did not appear; Cho / Cr and Cho / NAA ratio was significantly increased,metabolite vesicles edema lesions and normal brain tissue contralateral mirror Cho / Cr, Cho / NAA,there was a significant difference(P <0.05). 2 26 cases of edema and degeneration of the 1H-MRS showed lower NAA, Cho did not change significantly, Cr decreased significantly, eight cases of lesions appear inverted Ace, 5 cases of lesions appeare Lip,9 cases of lesions appear Gln; Cho / Cr and Cho/ NAA ratio increased, NAA / Cr ratio decreased. Degeneration edema lesions and normal brain tissue metabolites mirror Cho / Cr, Cho / NAA and NAA / Cr. there was a significant difference(P <0.05). 3 NAA degenerative lesions degree of edema reduction of lesions was significantly higher than the vesicles live insects; degeneration degree of edema lesions Cho vesicles live insects increased slightly below the lesion; degeneration reduce the degree of edema lesions Cr slightly above the lesion vesicles live insects. Two periods of peak lesions have appeared together Ace, Gln Lip, degenerative lesions appear edema Lac inverted, degenerative lesions and edema of live insects vesicles of NAA / Cr were significantly different(P <0.05).ConclusionSingle voxel 1H-MRS can objectively reflect parenchymal neurocysticercosis lesions metabolite information; while objective response to the different levels at different times within the lesion metabolites;Performance characteristics can be more rational staging the lesion based on the brain parenchyma cysticercosis 1H-MRS.
