Observation Of Color Doppler Ultrasonography In Children’s Renal Hemodynamics

Objective: To investigate the hemodynamic data of kidney main renal artery, segmental artery, interlobar artery and arcuate artery of normal children by color Doppler ultrasound measurements. And to analyze normal reference value of renal hemodynamics of children at different ages for the purpose of providing the valuable data for renal function assessment and for the clinical application to related diagnosisMethods: Peak systolic velocity, end diastolic velocity(Vd), resistance index(RI) and pulsatility index(PI) of renal arteries, segmental artery, interlobar artery and arcuate artery of normal children under 14 years who did physical examination in the hospital from March, 2013 to December, 2014 were measured by color Doppler ultrasonic measurements. The data then were compared to find out the differences of every parameter among different age groups by single factor analysis of variance. And regression analysis was used to analyze the linear relationship and the correlation between resistance index,pulsatility index and differentheigh,weight at differentages.Results: Renal hemodynamics of each vessel segment for a large sample of 400 normal children were measured by of the abdomen convex array probe and a small convex high frequency probe of color Doppler ultrasonic PHILIPSIU22 scan to investigate the changes of hemodynamics and RIrefrence value of different postures at different age groups. 1~3year RI 0.733±0.05、 PI 1.76±0.19;4~6year RI 0.701±0.040、 PI 1.61±0.31; 7~9year RI 0.688±0.015、 PI 1.45 ±0.27;10~14 year RI 0.679±0.019、PI 1.43±0.23. Renal vascular resistance declines with age and reaches adult level when thechildren are about nine years old. There are significant RI differences among different children groups withdifferent age,height,weight(p<0.05) RI in different gender, different position(prone position and lateral) and in lef tand right kidney between is not significant(p.0.05). The main renal artery, segmental artery, interlobar artery and arcuate artery,are closely related and there are significant differences among them(p<0.05).Conclusions: 1 normal children RI, PI values decrease with the increase of age, there were significant differences between different age group, about 9 years old reached the adult level.2 Renal vascular RI in different parts of the normal children, the PI value has the remarkable difference, the resistance of blood flow from high to low in the main renal artery, segmental artery, interlobar artery, segmental artery. 3There was no significant difference between normal children and renal hemodynamics in patients with RI, PI(P>0.05). 4 normal children RI, PI was negatively correlated with height, weight, decreased gradually with the increase of RI before the age of 9, the value of PI, about 9 years old reached the adult level. 5 normal children of RI, PI and gender, position, left and right kidney.
