Background and ObjectivesIncompletely obstruction of bile duct is a common disease in biliary tract diseases, itis caused by many kinds of factors, such as the inflammation of bile duct,calculus in bileduct,tumor,pancreatic disease,parasites, congenital biliary disease. So it can be lead toobstructive jaundice, and lead to disorder of water, electrolyte, and acid-base balance,endotoxemia, hyperbilirubinemia, and even cause damage to liver and kidney function andblood coagulation disorder in the body.long-term cholestasis that caused by benign biliarytract obstruction and chronic stenosis cholangitis all can eventually lead to the occurrenceof bile duct cancer. At present, the molecular biology mechanism of benign biliary tractobstruction lead to carcinoma of bile duct is not clear, but in the digestive tract malignanttumors, such as gastric cancer, colorectal cancer, esophageal cancer, especially in bile ductcarcinoma, the expression of Cox-2, Bcl-2protein the abnormally increased, and there is aclose relationship between expression of Cox-2, Bcl-2protein and tumor. So it is verynecessary to study the expression of these proteins in benign biliary obstruction. In thisstudy, The immunohistochemical technique was adopted for detecting the expression ofCox-2, Bcl-2and Fas protein in common bile duct epithelial cells of rats with incompletelyobstructive biliary tract at different period, and to observe the changes of Cox-2and bileduct epithelial cell morphology, and the regulation mechanism of apoptosis ofincompletely obstructive biliary tract, in order to explain whether Cox-2and Bcl-2caninduced carcinogenesis In the common bile duct epithelial cels in biliary obstruction.MethodsTotally60healthy Wistar rats of clean grade, Ignore the male and female, randomlydivided into two groups: A group40, common bile duct partial obstruction group, Furtherdivided into4sub groups, each sub group10only; group B20rats, as experimental control group, divided into4sub group, each sub group5. The rats in group A underwent commonbile duct ligation incompletely, to establish the animal model; Group B as thesham-operation rats. Each group of rats were sacrificed at postoperative day3,7,14and21,respectively. Then serum bilirubin levels were tested and common bile ducthistopathological changes were observed by HE. Apoptotis of common bile duct tissueswas assayed by TUNEL methed. The immunohistochemical technique was adopted fordetecting the Cox-2, Bcl-2and Fas Protein expression. Using SPSS17.0to analyze theresults, measurement data using x士s, Between the two groups were compared using Ttest, more than two groups were compared using the analysis of variance. P<0.05hadsignificant difference.Results1. The measurement of total bilirubin:In the experimental group (group A), with the time of incomplete obstructionprolonging, the values of serum bilirubin increased, There was no obvious change in groupB. The comparison between groupA and groupB had significant difference(P<0.05).2. The morphological changes of common bile duct epithelial cell:The common bile duct of the experimental group (group A) were dilated on the7thday after operation, and there were more inflammatory cells infiltration. The bile ductdilatation was more obvious on the21st day after operation, and bile duct wall thickening,fiber tissue and epithelial hyperplasia, there was no obvious change in group B.3. Changes of apoptosis level:In the experimental group (group A), apoptosis of bile duct epithelial cells increasedfrom the3rd day after operation. With the time prolonged, the apoptosis value wasincreased significantly. Apoptotic cells were rare in group B. The comparison betweenGroup A and group B had significant difference(P<0.01).4. The results expression of Cox-2, Bcl-2and Fas protein:In groupA, Cox-2protein showed weakly positive expression on the3rd day and the7th day, showed positive expression on the14th day and the21st day. Cox-2protein hadnegative expression in group B.In group A, Bcl-2protein showed weakly positiveexpression on the3rd day; on the7th day and the14th day, Bcl-2protein showed positiveexpression; on the21st day, Bcl-2protein showed strongly positive expression. But ingroup B, Bcl-2protein was almost no expression in the common bile duct epithelialcells.Fas protein showed weakly positive expression on the3rd day in group A, andshowed positive expression on the7th day and the14th day, and showed strongly positive expression on the21st day. In group B, Fas protein had also negative expression.Conclusions1. Apoptosis is an important mechanism of the body to keep dynamic balance, theexpression of Bcl-2protein and Fas protein can bregulate the apoptosis of common bileduct epithelial cells of rats with incompletely obstructive biliary tract.2. Cox-2protein and Bcl-2protein may be related to the proliferation of bile ductepithelium of rats with incompletely obstructive biliary tract.3. Proliferation of bile duct epithelial cells of rats with bile duct partial obstructioncan cause bile duct carcinoma common. The carcinogenesis may be produced by theinteraction of Cox-2protein and Bcl-2protein.
