Objective: To investigate the alteration of IFN-γ、IL-18and IL-18BP and their potentialclinical significance on the Th1/Th2imbalance caused in patients with hepatic cysticechinococcosis. Methods: The First group: Healthy control (HC,N=25),Liver cysticechinococcosis(CE,n=28)and Postoperative cysic echinococcisis(PCE,n=28); The Secondgroup: Grouped by cystic echinococcosis parting CE1(N=19)、CE2(N=21)、CE3(N=22)、CE4(N=18) and CE5(N=2);The third group: Detection of the ratio of IL-18and IL-18BPwith Th1/Th2cell factors correlation. For statistical analysis, LSD pairwise comparisonand Pearson correlation were used.Result:(1)Serum IL-18、 IFN-γ and IL-2concentrations were remarkably lower in CE compared with HC group(P<0.05).However,PCE group were remarkably higher than CE group(P<0.05);IL-18BP、IL-4、IL-9and IL-10concentration was higher CE compared with HC group(P<0.05),PCE werelower than CE groups(P<0.05).(2)The level of Th1and Th2factors was associated withthe ratio of IL-18and IL-18BP in CE patients.The group correlation coefficientwere(r=0.482,r=0.540r=0.506r=0.390r=0.400n=28),the different was statisticallysignificant(P<0.05).(3)Serum IL-18and IFN-γwere remarkably higher in CE1comparedwith HC group(P<0.05);were remarkably lower in CE2compared with CE1group(P<0.05); were no statistical significance between CE3and CE2group(P>0.05);were remarkably higher in CE4compared with CE3group(P<0.05). Serum IL-18BP wasremarkably lower in CE1compared with HC group(P<0.05); were remarkably higher inCE2compared with CE1group(P<0.05); were no statistical significance between CE3andCE2group(P>0.05); were remarkably lower in CE4compared with CE3group(P<0.05).Serum IFN-γ、IL-18and IL-18BP were no statistical significance between CE4and HCgroup(P>0.05).(4)Serum CRP、IgE、IgM and IgG concentrations were remarkably higherin CE compared with HC group(P<0.05). However,PCE group were remarkably lowerthan CE group(P<0.05);C3and C4concentration was lower CE compared with HC group(P<0.05),PCE were higher than CE groups(P<0.05). Conclusion: The imbalance ofIL-18and IL-18BP may one of the possible mechanisim that echinococcusgranulosusused to actively modulate and suppress host imm system so that Th1/Th2immune offset. Serum IL-18and IL-18BP in the development and progression of primaryCE,the role of collaboration and mutual restraint,the joint detection of all three can beused as a serum diagnostic marker of primary CE.Serum IL-18BP levels increased andIL-18to reduce closely related to the degree of malignancy of primary CE,joint detectionis more beneficial for primary CE staging to provide effective reference value. serumIL-18can kill EM, improve immune function,can be used as the clinical detection of CEAg killing activity and targeted therapy for CE patients to provide a potential target.
