Curative Effect Analysis——Prosthesis Replacement Of The Proximal Humerus

Objective:We collect and analyze these prosthesis replacement of the proximal humerus cases admitted to our hospital in the last five years, in order to study the surgical operative methods, operative effect, and complcation.Methods:We collect32cases of prosthetic replacement of proximal humerus tumor treated in our hospital from January1st2009to December31st2013, including13patients with osteosarcoma,6patients with chondrosarcoma,3patients with metastatic tumor,1patient with Ewing’s sarcoma,2patients with chondroblastoma,4patients with invasive and recurrent giant cell tumor of bone,1patient with giant cell tumor of bone associated with aneurysmal bone cyst,2patient with invasive vascular tumor. Perform the artificial humeral head prosthesis replacement, then postoperative functional evaluation score, curative effect analysis.Results:The follow-up for10-63months (median,34months) showed that3patients with osteosarcoma died in pulmonary metastasis after surgery,2cases of bone sarcoma in with postoperative Local recurrence presented in two patients with osteosarcoma and two patients with Giant cell tumor of bone,1patient had deep infection in the operational postion,2patients had shoulder subluxation after operation. There was no prosthesis loosening in any patients. Postoperative functional in MSTS was81.2%(51.4%-94.6%), abduction angle of the shoulder was56.5°(34°-78°), with extension35°(21°-48°), with circumgrate angle was26°(9°-43°) and abduction55.2°(22°-83°).Conclusion:The prosthesis replacement in limb salvage reconstruction is one of the most effective operation scheme for the treatment of proximal humeral bone tumor. To ensure operation safety of surgical resection to the boundary, protect shoulder joint function and surrounding soft tissue, early recovery of limb function, improve the life quality of patients.
