The thesis investigated the expression of Oct4in rectal carcinoma andcancer-adjacent tissues and their relationship among clinicopathologiccharacteristics, at the same time, studied further the influence on surgicalmargins and identified the right margins length.The samples of rectal cancer in hospital with80cases were from thedepartment of general surgery in the251sthospital of PLA from January,2011to may,2013. The great-sized paraffin block was taken, the sectionsin4μm thickness were kept in oven at65~70℃for1~1.5hours, and theexpression of Oct4in rectal carcinoma tissues, cancer-adjacent tissues andsurgical marginswere de tected by using EnVisionTMimmunohistochemistry in these samples. Cancer-adjacent tissues were farfrom tumor0.1cm mucous membrane, surgical margins were transversallyclipped2mm broad mucous membrane with80cases far from tumor1cm,2cm,3cm, which was respectively marked Group A, group B, group C.Control group D was rectal mucous membrane little chronic inflammationby rectal mucous membrane inflammatory polyp with40cases. All relateddatas were analyzed by χ2testing.1. The positive expression rate of Oct4in rectal carcinoma andcancer-adjacent tissues was respectively52.50%,30.00%,which wasstatistically significant between them(χ2=8.35,P<0.01). The positiveexpression of Oct4was related to histological differentiation, TNM stagingand lymph node metastasis(P<0.05), but not to gender, age, tumor size,carcinomatous infiltration depth and vasal cancer embolu(sP>0.05).2. Thepositive expression rate of Oct4in surgical margins A group and B grouprespectively was27.50%,22.50%, which was not statistically significant between them(χ2=0.53,P=0.46); The positive expression rate of Oct4insurgical margins C group and D group respectively was8.75%,7.50%,which was not statistically significant between them(χ2=0.05,P=0.81);The positive expression rate of Oct4in A group or B group was comparedwith C group, the difference was statistically significan(tχ2=9.47,P=0.002;χ2=5.73,P=0.02); The positive expression rate of Oct4in A group or Bgroup was compared with D group, the difference was statisticallysignificant(χ2=6.47,P=0.01; χ2=4.15,P=0.04).The abnormal expression of Oct4indicates the tumor stem cell ofrectal cancer is dysdifferentiation, which high-expression not only reflectsthe level of tumor stem cell, but also provides reference to some extent.
