Objective1. To study on distribution characteristics of FUO.2. To investigate clinic features of infective diseases in FUO.3. To investigate the clinic features of lymphoma in FUO.Methods1. Retrospectively analyze patient records of the Department of Fever Related Diseasesin Chinese PLA general hospital from December2009to December2010.503caseswere selected according to the diagnostic criteria of FUO. Disease categories wereinvestigated;Chinese literature in the etiology of FUO before and after10years ofchanges in the distribution were analyzed.2. Analyze the clinical data related with infective diseases;The clinic features of bacteriaand virus groups were compared;Investigate the clinic characteristics of tuberculosis.3. Compare the lymphoma to infectious diseases, connective tissue disease, adult-onsetstill’s disease(AOSD), and find clinical characteristics of lymphoma.Results1. Among503patients with FUO the most common is infectious diseases (51.2%),second is connective tissue disease (18.7%), followed by malignant tumors (8.9%) andmiscellaneous diseases (11.3%);Malignant tumor, Miscellaneous diseases, undiagnoseddiseases rate in male was higher than that of women, Elderly patients with malignanttumor is higher than that of young patients,The tumor ratio of less than20year-old wassignificantly lower than other groups;Nearly10years infectious diseases, connectivetissue diseases, miscellaneous diseases compared with previous10years has increased,but tumor disease and undiagnosed diseases ratio has reduced. 2. Through univariate and multivariate logistic regression that duration、weight loss offever before、 arthralgia、N、 ALP、SF entered into regression equation;Compared tobacteria, virus group are younger and the course are shorter, ALT, AST, LDH of virusgroup are higher than that of bacteria group,CRP, PCTof bacterial group are higher thanthat of virus group(P<0.05). The clinical characteristics of tuberculosis are Chills,weight loss, ESR, C reactive protein increased.3. Lymphoma temperature peak in the night or have no obvious rules, lymphoma groupof intermittent heat and weight loss ratio is higher than the other two groups(P<0.05);Lymphoma group of AST, LDH, SF were significantly higher than the othertwo groups, and TP, ALB were significantly lower than the other two groups(P<0.05);AOSD and lymphoma are difficult to identify.Lymphoma is more commonin men, Two groups of LDH were increased,LDH ALP of Lymphoma group is higherthan that of AOSD(P<0.05).Conclusion1. The study shows the etiology of FUO distribution and the proportion of the gender,age distribution in the etiology of FUO2. Find the factors associated with infectious diseases from the common clinicalfeatures,which is important for the diagnosis of infectious diseases in the guidance ofFUO. The study shows the index that meaningful in differential diagnosis of viralinfection or bacterial infection.3. The characteristics of fever, LDH, SF, ALP, TP, ALB can be used as an auxiliaryindex FUO in the diagnosis of lymphoma.
