Objective: To measure the concentration of leukaemia inhibitory factor(LIF)infollicular fluid on the day of the oocyte retrieval and in embryo culture medium on thethird day of in vitro fertilization and embryo transfer(IVF-ET),separately, to explore therelationship between the concentration of LIF infollicular fluid and embryo culturemediumwith embryo development and the clinical outcome of IVF-ET and to evaluatewhether LIF can be a marker of the quality of oocyte and transplanting embryo.Methods:76infertile women who were the first undergoing IVF-ET were divided intotwo parts.The first part which was divided into pregnant group and non-pregnant groupaccording to clinicl pregnancyincluded48infertile women with normal sexhormone. Thesecond partincluded13infertile women with PCOS as study group and15infertile womenwith normal sexhormone as control group. They received the same method of controlledovarian hyperstimulation(COH). Follicular fluid on the day of the oocyte retrieval andembryo culture mediumafter transplantationand freezing werecollected.The concentrationof LIF,estradiol (E2),progesterone (P) in follicular fluid and LIF in embryo culturemedium was measured to analysis the relationship among LIF,the quality ofoocyte,embryo and the outcome of IVF-ET and compare the difference of LIF betweenwomen in study group and control group.Results:1.The first part1.1The concentration of LIF in embryo culture medium of thepregnancy group was significantly higher than that in non-pregnancy group(P<0.05). Butthere was no difference of the level of LIF in follicular fluid between the two group(P>0.05).1.2The concentration of LIF in follicular fluid was positively correlated with thenumber of retrieval oocyte,the rate of mature oocyte,cleavage rate, high quality embryorate and E2(P<0.05).But it was notcorrelated with fertility rate and P(P>0.05).1.3Therewas no correlationbetween LIF of follicular fluid and LIF of embryo culture mediumaftertransplantation(P>0.05).1.4The concentration of LIF in embryo culture mediumof highquality embryo was significantly higher than that in poor quality embryo(P<0.05).2.The second part2.1Medication time of the study group was significantly longer thanthat of control group and dosage of medicationin of study group was also more than thatin control group (P<0.05).The number of retrieval oocyte of study group was more thancontrol group(P<0.05). But the rate of mature oocyte and high quality embryo rate of the study group was significantly lower than that of control group (P<0.05).2.2Theconcentration of LIF and P in follicular fluid of study group was significantly lower thanthat in control group(P<0.05),and the level of E2in follicular fluid of study group washigher than that of control group(P<0.05). But the level of LIF in embryo culturemediumhadno difference between the two groups(P>0.05).2.3The level of LIF of studygroup was positively correlated with the level of P in follicular fluid and the rate ofmature oocyte(P<0.05).Conclusions:1.The first part1.1The level of LIF in follicular fluid is correlated with E2and the quality of oocyte, and we can assess the quality of oocyte according to the level ofLIF. But the level of LIF in follicular fluid couldn’t predict the outcome of IVF-ET.1.2The level of LIF in embryo culture mediumincreases in pregnancy group and high qualityembryo group,so LIF in embryo culture mediumcan assess the quality of embryo andoutcome of IVF-ET.2.The second part2.1The level of LIF in oocyte fluid and embryoculture media of PCOS can not predict the outcome of IVF-ET.2.2The expression of LIFin oocyte fluid is lower in PCOS group,and the level of LIF is positively correlated withthe quality of oocyte.2.3The level of LIF of PCOS patient is positively correlated with P.3.The results of two parts all prove that both oocyte fluid and embryo culture mediumexpress LIF,but there was no correlationbetween them.
