ObjectiveTo evaluate the clinical and radiographic results of aconsecutive16cases (27hips) with ankylosing spondylitis(AS) whoundergone cementless total hip arthroplasty which performed indepartment of orthopedic of second hospital of jilin university fromJune2013to June2014.Data and methodsGeneral information: from June2013to June2014,16AS cases(27hips) did total hip arthroplasty in department of orthopedic ofsecond hospital of jilin university.14cases(24hips) were male,2cases(3hips) were female,average age was38.4years old(rangefrom22to62years old).3cases are right,2cases are left and11cases are bilateral.Average history of AS was17.5years(range from6-46years).Average duration of hip involvement to the initialsurgery is14.8years(3-46years).The way of the initial operation isTHA.8cases(16hips) undergo Bilateral synchronous THA;5cases(3hips) undergo unilateral THA;2cases(4hips) undergorevision for one side, THA for the other side;1cases(2hips) undergobilateral asynchronous THA. All hips are non-cemented.Methods: All of the patients using cementless THA.respectively, in the preoperative and postoperative3month,6 months,last follow-up of patients using Harris score hip functionalscore,VAS scores and hip range of motion compared thepreoperative of Harris score,VAS score, range of motion withpostoperative3-month,6-month last follow-up, as well asmeasurement of inclination and anterversion of the cup throughX-ray after surgery evaluate the short-term curative effect of THAforAS patients with the help of SPSS,.ResultsThe average follow-up time of16cases was15.5months(rangefrom12months to24months).the preoperative average harris scoreis42.51±15.47and the postoperative3-month average harris score is82.59±9.84,6-month is88.11±9.84,12-month visit is91.37±9.19.The preoperative average VAS score is3.70±3.09and thepostoperative3-month averge VAS score is0.74±0.89,6-monthvisit is0.37±0.67, the12-month follow-up visit is0.33±0.54. Thepreoperative average range of motion of the hip is47.22±40.26,3-month visit is86.74±22.19,6-month visit is92.59±19.05and the12-month visit is103.07±19.35. All have significantlyimprovement, all datas show statistically significant. Inclination ofthe cup is40.06±10.1, anterversion of the cup is17.0±6.2.According to the harris score among them,14hips are A(100-90),11hips are B(89-80),1hips are C(79-70),1hips are D(<70),thefineness rate is92.6%.ConclusionCementless total hip arthroplasty is the most effective treatment for hip involvement in AS patients,which could greatly improvehip joint function,relieve hip pain and improve the quality of life.For AS patients with hip bony ankylosis or fibrous ankylosis, itis quite important for two-stage femoral neck osteotomy andprosthesis functional set to improve surgical safety and preventdislocation.
