ObjectiveTo investigate the incidence trend and clinical features of prostate cancer in Chinese Men, a retrospective analysis of 725 patients with prostate cancer for 14 years in a single center was carried out. And then, to investigate the role and significance of Partin tables in predicting the staging of Chinese prostate cancer and supply the decision-making basis for the scientific diagnosis and treatment, we also validated the accuracy of the four versions of 1997, 2001, 2007 and 2012 Partin tables in predicting the postoperative pathological characteristics of prostate cancer.Methods1. All the clinical data of 725 patients of prostate cancer diagnosed and treated in our hospital was collected and analyzed by Excel 2010, including the number of prostate cancer, age and PSA at diagnosis, the biopsy Gleason score, clinical staging and final pathological staging.2. We retrospectively analysed the data of 163 patients with prostate cancer underwent radical prostatectomy in Daping hospital between February 2005 and December 2013. According to the four versions of 1997, 2001, 2007 and 2012 Partin tables, we achieved the probability of the final pathological characterstics(OC, EPE, SVI and LNI) of prostate cancer after the radical prostatectomy, respectively.3. Four versions of the Patirn tables were compared for accuracy and performance to predict final pathological staging using receiver operating characteristic curve and are a under curve by SPSS19.0 software.Results1. The numbers of the patients with prostate cancer were 15,15,22,10,16,20,31,32, 40,42,76,88,90,110 and 118 in each year from 1998 to 2013(without 2000).2. Among 725 cases with prostate cancer, we analysed them by different criteria. Regarding to PSA, PSA ≥20 ng/ml and PSA 10-20ng/ml accounted for 64% and 14%, respectively; while only 9% was PSA <4 ng/ml. Concerning Gleason scores, the numbers of each group(Gleason2-6, 7 and 8-10) were 136(29%), 163(35%) and 171(36%), respectively. With respect to clinical stages, the numbers of each group(T1+T2,T3+T4 and metastasis) were 330, 63 and 235. As to risk stratification, the numbers of low-risk, intermediate-risk, high-risk and metastasis were 34, 47, 206 and 161.3. The area under the receiver operating characteristic curve of the Partin tables 1997, 2001, 2007 and 2012 was 0.749, 0.725, 0.709 and 0.665 for OC; 0.503, 0.461, 0.504 and 0.607 for EPE; 0.749, 0.603, 0.504 and 0.607 for SVI; 0.718, 0.698, 0.632 and 0.797 for LNI, respectively.Conclusion1. The incidence of prostate cancer in Chinese Men was increasing, which included two apparent phases : the slow growth from 1998 to 2005 and the rapid growth from 2006 to 2013.2. We also found that most of the patients were at advanced stage or intermediate and high risk at the time of diagnosis; more than half of the patients missed the chance of radical prostatectomy, which suggested that the application of PSA screening in prostate cancer should be increased instead of being restricted in Chinese Men, so that most prostate cancer patients could be diagnosed at earlier stages.3. All four versions of the Partin tables showed a poor accuracy to predict EPE. 1997, 2001 and 2012 showed a good accuracy to predict OC. The 1997 and 2012 Partin tables revealed superior accuracy in predicting SVI and LNI. However, in predicting LNI, the version of 2012 Partin tables was much better than the other three versions.4. Our findings suggested that the combination of 1997 and 2012 versions of Partin tables could improve the accuracy in predicting final pathological staging of prostate cancer.
