ObjectiveRetrospective analysis of the treatment and follow-up of 182 cases of prostate cancer patients with clinical data,diagnosis and treatment of prostate cancer at the preliminary stage summary made in our department,to investigate the effect of different castration therapy for prostate cancer,a brief analysis of the relationship between PSA before treatment,clinical stage,Gleason score and castration resistant prostate cancer in patients with castration resistant prostate cancer,the relationship between PSA before treatment,clinical stage and Gleason score of three patients with castration resistant prostate cancer,which provide reference for the clinical diagnosis and prognosis of prostate cancer.Methods1.Collect and analyze the data of 182 patients with prostate cancer who were followed up at the Department of Urology of the Second Affiliated Hospital of Nanchang University from January 2008 to December 2015 were collected and followed up until March 2017.2.The data of patients who were followed up were analyzed,and the diagnosis and treatment of prostate cancer in our department were summarized.This study uses SPSS21.0 software for data processing.Frequency of descriptive statistics,Mean ±standard deviation.The comparison of castration resistance rate can be made by chi square test.The comparison of the time of castration resistance with t test,P < 0.05 for the difference was statistically significant.The relationship between PSA before treatment,clinical stage,Gleason score and castration resistance in patients with prostate cancer was examined by Logistic analysis.Results:1.182 cases of prostate cancer patients,the average age was 71.62 + 8.01,minimum 41 years,maximum 89 years old;the minimum value of PSA is 0.07ng/ml,the maximum value is 3126ng/ml;the Gleason score was 7.56 + 1.26,minimum 4,maximum 10 points;in the stage of tumor,T1 patients were 41 cases,stage T2 patients were 56 cases,stage T3 patients were 41 cases,stage T4 patients with a total of 44 cases;2.Radical prostatectomy was performed in 35 patients with an average follow-up of 33.25 ± 12.79 months.The recurrence rate was 18.2 ± 5.4 months in all the 6 cases.Surgical castration was performed in 97 patients,with an average follow-up of 36.14 ± 1.71 months.All patients developed castration resistant prostate cancer(n = 24).The average duration of castration resistance was19.62 ± 10.90 months.41 patients were followed up for 27.19 ± 10.14 months with an average of months,and castration resistant prostate cancer was observed in all patients.The average time of castration resistance was 18.81 ± 9.26 months.9 patients(including radiotherapy and chemotherapy,untreated patients)were followed up for27.88 ± 15.23 months;3.The trend of castration resistance in two different treatment methods,surgical castration and drug castration,was the same(P > 0.05);4.There was no difference between the two methods of operation castration and drug castration in the treatment of prostate cancer;5.There was a positive correlation between PSA value before treatment,Gleason score and clinical stage in patients with castration resistant prostate cancer.Conclusions1.Prostate cancer patients are mostly elderly patients with an average age of71.62 years;2.Most of the patients were in the advanced stage,the majority of patients with II-IV;3.In the treatment of patients with advanced prostate cancer,the effect of drug castration or surgical castration is similar,and can be decided according to the patient's wishes and physical condition.4.The treatment of Castration prostate cancer patients,the higher the PSA before treatment,the more likely to progress to CRPC;the higher the clinical stage,the more likely to progress to CRPC;the higher the Gleason score,the more likely to progress to CRPC.
