| Objectives1. To analyze the dynamic trend of HIV epidemic among college students in Kunming during 2010-2014 based HIV sentinel surveillance.2. To analyze knowledge and behavior of AIDS and their influencing factors of students in a college during 2010 to 2014, and provide information for sexual health education and behavioral intervention among college students in Yunnan.3. To provide a reference to improve HIV sentinel surveillance work of young students in Yunnan.MethodsAccording to the guidelines for national sentinel surveillance, cluster sampling method was used. Graduated students from Kunming were recruited to fill out questionnaires involve demographic information, knowledge of AIDS, behavioral characteristics. And their blood was collected to conduct the tests of HIV, HCV and syphilis in Yunnan Center for Disease Prevention and Control. The data were entered into the national sentinel surveillance system, and using SPSS 17.0 software to analyze. Analysis of variance,x2-test were used in univariate analysis, binary logistic regression analysis was used in multivariate analysis.Results1. Laboratory results:of the 3938 participants, nobody was HIV positive, there were 2 HCV positive and 1 syphilis positive, their infection rate was 0.05% and 0.02% separately during 2010-2014.2. The condition of AIDS/HIV knowledge:the awareness average rate of AIDS/HIV knowledge was 97.6%, each year there were 97.2%,99.3%,99.7%,97.6% and 94.3% respectively and it had statistical difference in the awareness rate of AIDS/HIV knowledge during five years (x2=63.168, P<0.001). "May sharing syringes with HIV infection be infected with HIV? ", its accuracy was highest in 8 questions about AIDS/HIV. The lowest accuracy of these questions was "Can a mosquito’s bite deliver HIV to the body?". As shown by univariate and multivariate analysis, the awareness rate of AIDS/HIV knowledge was associated with gender, AIDS/HIV advocacy services.3. The status of sexual behavior:the average incidence of sexual behavior among college students was 10.0% during 2010-2014, each year the rate were 3.3%, 12.7%,15.8%,11.5% and 7.2% respectively, it had statistical difference during five years(x2=85.047, P<0.01).The average age of first sex was 20.74±2.17 years old.In the last year,the proportion of the students had sex with fixed sexual partner was 61.2%, there respectively were 50.0%,72.6%,77.4%,56.9%,34.0% from 2010 to2015, it had statistical difference (x2=8.594, P=0.035). The proportion of casual sexual behavior was 10.1%, there were 16.7%,11.9%,9.0%,7.2% and 10.6% respectively in five years, it had no statistical difference (x2=3.829, P=0.430). The proportion of commercial sexual behavior with commercial sexual partner was 1.7%, there were 0.0%,3.6%,0.0%,1.8% and 2.4% respectively in five years, it had no statistical difference (x2=5.571, P=0.152).The proportion of same-sex behavior among male students was 0.9%.The incidence of sexual behavior among male students was 19.1% and 5.2% to the female students, it had statistical difference in them (x2=185.846, P<0.01), male students’ sexual incidence was higher than the female students’. The incidence of sexual behavior among age group of <20years was 2.6%,20~25 years was 10.5%,>25 years was 16.6%, it had statistical difference in them (x2=23.372, P<0.01) and showed increasing trend by linear trend x2-test. The sexual incidence of students came from Yunnan province was 9.8%,15.3% to the non-Yunnan province, it had statistical difference in them (x2=10.209, P<0.01). The students came from non-Yunnan province likely had sexual behavior. The sexual incidence of witting students in AIDS/HIV knowledge was 10.1%,17.9% to the non-witting students, it had statistical difference in them (x2=5.304, P<0.05). In multivariable logistic regression, the incidence of sexual behavior was associated with gender, age and the household register.4. The condition of receiving or participating AIDS/HIV advocacy services:in recent one year, the proportion of receiving AIDS/HIV advocacy services was 60.5% during 2010-2014, there were 51.9%,64.1%,68.8%,64.2%,53.8% respectively in five years, and it had statistical difference in them(x2=71.410, P<0.01). The proportion of participating AIDS/HIV advocacy services was 26.1% in five years, each year there were 17.1%,29.0%,31.1%,25.5%and 27.7% respectively, it had statistical difference in them(x2=48.463, P<0.01),there showed increasing trend in 2010-2012 and declining trend in 2013-2014 (x2=13.579, P<0.01) by linear trend x2-test.Conclusions1. Yunnan province was the area with high-incidence of AIDS, college students were the risk population for AIDS, sentinel surveillance work among college students was profound and lasting.2. The overall level of the awareness rate of AIDS/HIV knowledge among college students was very good, but the level in non channels of infection was lower. The knowledge rate among male participants and students who didn’t receive AIDS advocacy services in the last year were lower.3. There is much less awareness of AIDS/HIV among college students, the proportion of condom use was low in sexual behavior universally.4. The incidence of sexual behavior was associated with gender, age and the household register, and also associated closely with attitude, family.5. Health education is an effective method on HIV/AIDS prevention and control at present, it is much important to construct and perfect the platform of health education. |
