A Study Of Normal Dilivery Related Factors Of Single Disease’s Hospitalization Expenses Composition In A Third Grade First-class Hospital

ObjectivesThrough statistical analysis of normal delivery hospitalization expenses of single diseases related factors in a third Grade first-class Hospital of Yunnan province from 2008 to 2012, we understood the composition of hospital cost about normal delivery, change trend, and its influencing factors. After the factors of hospitalization expense unreasonable growth were analyzed,we put forward some reasonable ideas to control the costs of hospitalization, which would provide a scientific basis for price formation mechanism of single diseases,and scientific advice for the local administrative departments of medicine and health care and medical insurance institutions to develop payment standard of normal dilivery of single disease.MethodsBy using retrospective study, the third Grade first-class Hospital of Kunming, yunnan province was chosen,1,843 valid data of normal dilivery effective medical records of single diseases were collected in this hospital from January 2008 to December 2012. All the data were set up a database in Epidata3.1, after proofreading, all data were summarized, sorted and analyzed through SPSS 18.0 statistical software.Results1, From 2008 to 2011, normal dilivery hospitalization expenses of single diseases showed an increased trend year by year,but, from 2011 to 2012, there was a decreased trend in the total hospital cost, accompanied by drug costs down.2, Cost of every item,medical insurance group were higher than the cost themselves.3, The number of episiotomy operation accounted for 61.4% of the total, but the proportion is lower since 2010. The average hospitalization expense of episiotomy is higher than the non-episiotomy.4, The total cost of normal delivery single diseases increased year by year. The results showed that the expenses of medicine, surgery, examination,and treatment had been accounted for most of the cost, the cost of medicine was main.5, From the standardized regression equation results, we can see that the main factors influencing of the hospitalization expenses from major to minor was:the mode of payment, number of delivery, with or without secondary diagnosis, the actual days in hospital, past medical history, days of postoperative, career.Conclusions1, The unreasonable growth of hospitalization expense should be controlled.2, Cost of every item,medical insurance group were higher than the cost themselves. Therefore, we should improve the current medical insurance system step by step, establish and improve single disease payment of medical system, and control the unreasonable growth of medical expense.3, The average hospitalization expense of episiotomy is higher than the non-episiotomy.Therefore, it was more important to reduce rate of episiotomy for decreasing the hospitalization expense.4, The average hospitalization day of normal dilivery was 4.5 in China, the average hospitalization day was 3.9 in city. According to the analysis, the average hospitalization day in this hospital was below the national average level, it shown that administration was excellent. Average hospitalization day was affected by many factors, shorten the average hospitalization day can reduce effectively the hospitalization expenses.5, The hospital resources should be allocated rationally, to control the unreasonable growth of medical expense,we should control the reasonable proportion of drugs expenses, reduce repeated and unnecessary tests.
