Object:To compare the effects of early fluid resuscitation in septic shock under the guidance of CVPpressure monitoring and PICCO capacity monitoring.Method:49 patients with septic shock were admitted to EICU of the First Affiliated Hospital of Kunming Medical UniversityfromJanuary 2014-January 2015,were enrolled into our trial, these patients were randomly divided into experimental group and control group.On the basis of the treatment of anti-infection, nutritional support and so on,at the same time,they were treated in the guidance of EGDT bundls.In the process of the treatment of EGDT bundls,the experimental group used the PICCO capacity monitoring to guide the fluid resuscitation, and the control group used the CVPpressure monitoring to guide the fluid resuscitation.At last, comparing the effects and outcomes between the two groups of patients who accepted the treatment of fluid resuscitation.Results:1.The two groups of patients had no statistical difference in general condition. (P>0.05)2.The lactate clearance rate and resuscitation goal of the experimental group was higher than the control group’s after the resuscitation in 6h,12h,24,48h,72h,there are significant differences (P<0.05)3.28-day mortality rate,MODS incidence,6hEGDT standardof the experimental group were higher,the time of usingvascular active pharmaceutical,duration of mechanical ventilation and ICU stay dayswereless than the control group’s, there are significant differences (P<0.05)Conclusion:1.PICCO capacity monitoring technicalguidance septic shock the fluid resuscitation is more precisely than CVPpressure monitoring guidance the fluid resuscitation in patients with septic shock,in terms of fluid management,reaching higher 6hEGDT standard; reduce the time of usingvascular active pharmaceutical, shorten duration of mechanical ventilation and ICU stay days; improving lactate clearance rate and reducemortality.2. Lactate clearance rate has prognostic significance in early fluid resuscitation in septic shock.
