Objective:We aim to access the clinical efficacy of retroperitoneal laparoscopic radical nephrectomy in treating localized renal cell carcinoma, compared with open radical nephrectomy, evaluating its clinical values, as well as its feasibility in treating localized renal cell carcinoma in middle-small hospital. Methods:Between February 2009 and February 2015, a total of 63 patients with localized RCC underwent radical nephrectomy in our hospital. We divided them into two groups according to the surgery options, the case group whom received retroperitoneal laparoscopic radical nephrectomy and the control group whom received open radical nephrectomy, and conducted a comprehensive comparison in the terms of operation time, amount of bleeding, hospital days, recovery of bowel function, volume of drainage, surgery cost and complications during or after the operation for these two surgical options. Results:A total of 31 and 32 patients received retroperitoneal laparoscopic radical nephrectomy (case group) and open radical nephrectomy (control group) separately. However, we identified that the amount of bleeding was significantly decreased (90.00+65.00 ml,154.00+116.00 ml, P<0.01) when compared with control group, as well as the hospital days (9.0+2.0 d,5.0+3.0 d, P<0.01) and the volume of drainage (65.0±20.0 ml,120.0+30.0 ml, P<0.01). In addition, the removal time of the drainage tube was earlier (3.0±1.0 d,5.0+2.0 d, P<0.01), whereas the surgery cost was higher (11385.47+980.0 RMB,7437.31±520.0 RMB, P<0.01), when compared with the control group. No significant difference was identified for the recovery time of bowel function and the operation time for these two groups. Moreover, the incidence of the complication rates was also statistically significant when compared with the control group, except for pneumoderm (P<0.05).Conclusions:When Compared with traditional open surgery option, we identified that retroperitoneal laparoscopic radical nephrectomy is advantageous clinically in terms of the less amount of operative bleeding, the shorter hospital stay, and lower rate of complications and etc. Therefore, it has superior medical research values in clinical research and is appreciated to propagate and promote in middle-small hospital.
