Objective:This study analyzed the changes of index of coagulation function ininflammatory bowel disease (IBD) active stage, further to explore therelationship between the related indexes of blood coagulation function and theactivity of the disease, and provide a reference for the assessment and treatmentof the disease.Methods:The research objects are selected from2008January to2014December inNo.1Hospital of Jilin University,and they all had a definite diagnosis ofulcerative colitis or Crohn’s disease and in the active phase of disease. There are84patients in total, of which the clinical data are complete, the diagnosis areclear. First, they are divided into group UC and group CD, and then according tothe modified Truelove and Witts inflammation degree classification standard,group UC patients were divided into3groups which are mild, moderate andsevere groups separately. Also according to the clinical disease activity index ofUC scale for assessment of disease activity index(CAI). At the same time thepatients of CD are assessed disease activity index (CDAI)with Crohn’s diseaseactivity index table, also divided into mild, moderate, severe groups. ChooseNo.1Hospital of Jilin University100healthy people as control group. Peripheralblood thrombin time(TT), the activated partial thromboplastin time(APTT),prothrombin time(PT), prothrombin activity(PTA), fibrinogen level (FBG)andplatelet count(PLT), mean platelet volume (MPV)of case group are comparedwith these of control group, and Compare the related indexes of coagulation of different severity of groups in UC and CD respectively. Analysis of therelationship between the coagulation parameters and disease activity index orerythrocyte sedimentation rate(ESR) in UC group and CD group, usingSpearman correlation analysis and multiple regression. Statistical analysis usingSPSS17.0software, P<0.05showed a statistically significant, while P<0.01showed that there was significant difference.Results:1、In group UC, PLT、FBG、PT (all P <0.01) was significantly higher thanthe control group, while PTA、MPV (P <0.05) lower than the control group. Ingroup CD,PT (P <0.05) was higher than the control group, and PLT、FBG (P<0.01) was significantly higher than the control group, MPV (P <0.05)was alsolower than the control group. TT、 APTT difference was not statisticallysignificant. In UC and CD groups,each index between the two groups was notstatistically significant.2、Mild patients and moderate patients of group UC were compared, PT(P<0.05) of mild patients was lower than that moderate of patients,and PLT、FBG (P<0.01) was significantly lower than that of moderate patients, PTA(P<0.01) was significantly higher than the latter; In the patients with severe UC,PT、PLT、FBG (P<0.01) was significantly higher than that of mild patients,PTA、MPV(all P<0.01) was significantly lower than that of mild patients, andMPV (P<0.05) is also lower than that of moderate patients3、Mild patients and moderate patients of group CD were compared,FBG(P<0.05) of mild patients was lower than that of moderate patients,and PLT(P<0.01) was significantly lower than that of moderate patients. In the patientswith severely active CD, MPV (P<0.05) was lower than that of mild patients,FBG(P<0.05) was higher than that of mild patients, and PLT(P<0.01) wassignificantly higher than that of mild patients, 4、A significant positive correlation between PLT and FBG (P<0.01) ofpatients in the group UC, PLT、FBG and CAI showed a significant positivecorrelation (P<0.01), PT was positively associated with CAI, MPV wasnegatively correlated with CAI (P<0.05); there is significant positivecorrelations between PLT, FBG and ESR (P<0.01), PT is positively correlatedwith ESR (P<0.05). There was a linear relationship between PLT and CAI(P<0.05), was linearly related to PLT, FBG and ESR (P<0.05).5、There was a positive correlation between PLT and FBG (P<0.05)of thepatients in the group CD, PLT、FBG were positively correlated with CDAI(P<0.05), MPV was negatively correlated with CDAI (P<0.05), PLT ispositively correlated with ESR (P<0.05), MPV was negatively correlated withESR (P<0.05). There was a linear relationship between MPV and CDAI or ESR(P<0.05).Conclusion:1、During the active period of IBD the function of blood coagulation andfibrinolysis abnormalities is abnormal, manifested as elevated platelet count,mean platelet volume decreased, PT prolonged, and FBG increased, suggestingthat immune hypercoagulable state, and with the severity of disease changingthey would show changes.2、There is a correlation between the platelet count, mean platelet volume,fibrinogen, prothrombin time and disease activity index or ESR in UC patients.At the same time the platelet count has a linear correlation with disease activityindex and ESR, and it could be used to determine the UC disease activity index.3、There is a correlation between the platelet count, mean platelet volume,fibrinogen and disease activity index or ESR in CD patients. At the same timemean platelet volume has a linear correlation with disease activity index andESR, and it could be used to determine the CD disease activity index.
