Through Hemi-semilaminectomy Microsurgical Operation Treatment Of14Patients With Spinal Dural Arteriovenous Fistula (SDAVF)

ObjectiveTo explore the hemi-semilaminectomy microsurgical operation treatment of spinal dural arteriovenous fistula (SDAVF) is effective and feasible.Provides a new minimally invasive method for the treatment of spinal dural arteriovenous fistula of the animals.MethodsRetrospective analysis clinical data from32patients with SDAVF from2006January to2014January who diagnosed in my department.Before operation all patients were diagnosed by spinal MRI and spinal angiography, Of which27cases of male,5cases were female, male to female ratio was5.4:1, the average age is51.43years old, the average duration of12.6months.14patients were treated by hemi-semilaminectomy approach and other18patients were by traditional laminectomy approach,however. Both datas were analysised by ttest. ResultsThe nidus of SDAVF located on thoracic segments in18, lumbar segments in12, Cervical segment4, which was fed by single artery.14patients were treated by hemi-semilaminectomy approach and the mean operative time was 101.86minutes, average blood loss was81.43ml, the average postoperative hospital stay was13days, average hospitalization cost was24284.36yuan. Other18patients were by traditional laminectomy approach,however Some patients in our department in patients with minimally invasive operation before, some patients can not the precise location of fistula and fistula located in the cervical segment,and the18cases of patients with the average operation time was138.61minutes, average blood loss was130.56ml, the average postoperative hospital stay was16days, average hospitalization cost was28098.89yuan. Both datas were analysised by T test and P<0.05. There was significant difference. The hemi-semilaminectomy microsurgical operation showed shorter operation time, less bleeding, less hospitalization time and cost compared with traditional operation mode. Follow-up of all patients for6months-2years all patiens, symptoms were improved compared with before surgery.ConclusionSpinal dural arteriovenous fistula (SDAVF) is a type of vascular malformation of the spinal cord in the most common and the clinical manifestations of spinal dural arteriovenous fistula animals lack of specificity, the first choice in the diagnosis is DSA.Operation treatment of spinal dural arteriovenous fistula is a kind of effective method.
