Objective Migraine,,which is a recurrent multi lateralization, moderate to severe and throbbing headache,is the most common primary headache. Symptoms of headache which can be induced or aggravated by acousto-optic stimulation, fatigue and other factors often accompanied by malignant, vomiting, photophobia, phonophobia symptoms.The high incidence of migraine headaches and severe pain, can seriously influence the patient’s life, work, learning. Some research suggests that sleep of 40% of migraineurs sufferers have been seriously affected and about 21% of migraine sufferers have mild sleep, resulting in a series of harm and reduce the quality of life and social function in patients.The attack of migraine can seriously affect the patient mood,depression associated with increased stroke risk, etc. Many investigations showed that migraine has a variety of comorbidity,including epilepsy, depression, CADASIL,cerebral infarction,etc.Although there are many studies in migraine, its pathogenesis is still not very clear. In recent years, the trigeminovascular theory attracted widespread attention.The theory suggests that sterile neurogenic inflammation is the pathophysiological basis of migraine and substance P, calcitonin gene related peptide(calcitonin gene related peptide, CGRP), 5-HT, nitric oxide(NO) and so on the many kinds of inflammatory mediators have been considered involved in the pathogenesis.So do the interleukin-1(IL-1), interleukin-6(IL-6), tumor necrosis factor alpha(TNF- alpha) and other inflammatory cytokine abnormality.Cyclooxygenase-2(COX-2) catalyzes the conversion of arachidonic acid to prostaglandins peanut four E2(prostaglandin E2,PGE2) and is the rate limiting enzyme,which the expression is high in middle meningea in some migraine animal modell.Visfatin is a new adipocytokines, originally discovered in lymphocyte secretion before B cell clone factor A(pre-B cell colony-en-hancing factor, PBEF), and then was found exists widely in human and mouse visceral adipose tissue, and the serum levels were positively related to human obesity, hence was named visfatin. But its exact position remains controversial, it can be generated in multiple sites of vascular endothelial cells. At present, the role in inflammatory has attacted gradually attention and more and more extensive research. We hope to explore the pathogenesis and related process of migraine in further and prevention and treatment of migraine in the new target,throuh the study serum levels of COX-2 and Visfatin of migraineurs.Mthods We take 61 patients as case group which is collected from 2014 January to 2015 January into the Liaoning province people’s Hospital for emergency treatment and hospitalization after diagnostic criteria and exclusion criteria of migraine.The case group is divided into two groups which the 22 of the total patients was aura group and the 39 of the total patients was aura group. By case-control association analysis.Testing the serum levels of COX-2 and Visfatin using Enzyme Linked Immunosorbent assay(ELISA),and then analysis the data with the SPSS17.0 statistical analysis software.Results1.The serum levels of Visfatin in migraineurs are significantly higher in comparison to the levels of the healthy controls(Z=1.969,P=0.001);The serum levels of COX-2 in migraineurs are significantly higher in comparison to the levels of the healthy controls(Z=3.405,P=0.000).2.The serum level of COX-2 in migraine with aura(MA) and in migraine without aura(MO) were no difference;The serum level of Visfatin in migraine with aura(MA)and in migraine without aura(MO) were no difference.3. Correlation analysis shows there is no significantly correlation between serum levels of COX-2 and Visfatin.Conclusion1.The serum levels of Visfatin and COX-2 in migraineurs are significantly higher in comparison to the levels of the healthy controls.suggesting that COX-2 and visfatin is involved in the pathomechanism of migraine.2. There is no significant difference between the two groups in MA and MO.Although both of COX-2 and Visfatin are involved in the pathomechanism of migraine,there is no specificity between them.3.There is no combined action between COX-2 and Visfatin in both MA and MO.
