Objective: With the arthroscopic findings as diagnostic standard, to analysis the value of the different sequences of MRI for acute injury of anterior cruciate ligament and meniscus.Methods: From January 2012 to August 2014, in the Second Affiliated Hospital of Dalian Medical University,the clinical data and MRI image data of 36 patients with chief complaint of Injury of knee joint were selected,and in these cases,arthroscopic examination was used in the 1-10 days after 3.0T MRI diagnostic techniques.Counted the cases of injury lateral and medial meniscus as well as anterior and posterior angle and injury anterior cruciate ligament in the MRI images according to damage degree.Based on the results of arthroscopy, calculate the sensitivity, specificity and accuracy of different MRI sequences in the diagnosis of meniscus tear, anterior cruciate ligament rupture.Using SPSS software for data to carry on the statistical analysis.Withχ2test to analysis the different between MRI sequences of the meniscus, anterior cruciate ligament injury diagnosis.With the Kappa test to analysis the consistency of MRI inspection and arthroscopic diagnosis of meniscus tear and anterior cruciate ligament rupture.Results:There are totally 148 menisci counted seperately by lateral and medial meniscus as well as anterior and posterior angle.The images on SE T1 WI showed that111 menisci were of level 0,10 of levelⅠ, 24 of level Ⅱ, 3 of level Ⅲ. The images onFSE PDWI fat suppressed sequence indicated that 91 menisci were of level 0, 7 of levelⅠ, 42 of level Ⅱ, 8 of level Ⅲ.The χ2show that there is statistical significance in SE T1 WI sequence and FSE PDWI sequence for diagnosis results of meniscus injury of knee joint difference(P<0.05).The Kappa test show that MRI SE T1 WI and arthroscopy of meniscus tear results poor consistency(kappa = 0.333), MRI FSE PDWI and arthroscopy of meniscus tear have a high degree of consistency.(kappa = 0.609). There are totally 37 cases of anterior cruciate ligament.The images on SE T1 WI diagnosis of injury in 25 cases, 16 of levelⅡ, 9 of level Ⅲ. The images on FSE PDWI diagnosis of injury in 31 cases,11 of levelⅠ, 8 of level Ⅱ, 12 of level Ⅲ.The χ2test shows that there is statistical significance in SE T1 WI sequence and FSE PDWI sequence for diagnosis results of anterior cruciate ligament injury of knee joint difference(P<0.05);The Kappa test shows that the results of ACL rupture of MRI SE T1 WI and arthroscopic diagnosis showed moderate consistency(kappa = 0.507), the results of ACL rupture of MRI FSE PDWI and arthroscopic diagnosis have a high degree of consistency.(kappa = 0.6773).Conclusion: MRI FSE PDWI in the diagnosis of acute knee joint injury of meniscus tear, anterior cruciate ligament rupture with arthroscopy result has a high degree of consistency. In the diagnosis of meniscal tear or rupture of the anterior cruciate ligament,FSE PDWI fat suppression sequence is preferred than SE T1 WI. Although MRI examination results in false positives and false negatives,but it is still an effective diagnosis method of meniscus remains and anterior cruciate ligament injury in the acute injury of knee joint.
