Objective: Based on the principle of tonifying kidney and strengthening bone, we observed the biochemical indexes of bone metabolism, bone density, estrogen and TCM syndrome effect on postmenopausal osteoporosis in clinical by modified Erxian Decoction Combined with alendronate. We compare our method with only alendronate, exploring the approaches to improve curative effect of postmenopausal. Finally, the experimental results provide the evidence that tonifying kidney and strengthening bone is useful for prevention and treatment of postmenopausal osteoporosis. Methods:We choose 40 cases of willing to join clinical trials and clinically diagnosed as postmenopausal osteoporosis, kidney yin-yang deficiency in Chinese Medicine. In accordance with the principle of randomization,40 cases were divided into group A (combination treatment group) and group B (alendronate). Group A was treated with modified Erxian Decoction combined with alendronate, and group B was treated with alendronate. Observe the TCM syndrome score after the treatment of different stages; and measure the bone density, serum estradiol and bone alkaline phosphatase and tartrate resistant acid phosphatase before treatment and after 12 weeks of treatment, with statistical analysis and comparison. Results:1. In TCM syndrome integral, compared with the results before and after treatment in the same group, the results from two groups were notablly meaningful; the outcome after therapy from different groups were also statistically significant (P< 0.001). Compared with the integrated effect of the two groups, group A was 50%, effective 45%, invalid 5% and the total effective rate was 95%; group B was 0%, effective 75%, invalid 25% and the total effective rate was 75%. The results show that group A can significantly improve Clinical symptoms of traditional Chinese Medicine and the total effective rate.2. In the aspects of relieving backache, soreness and weakness of waist and knees, warm sweating et al., the difference was statistically significant (P< 0.01) in the group A before and after treatment, and no significant difference (P> 0.05) was observed in the group B before and after treatment. In relieving warm sweating, there was statistical difference (P< 0.05) between two groups after treatment.3. Compared with BMD, E2, B-ALP and TRACP5b between group A and group B before treatment, P> 0.05, which means there was no statistical significance, and it was reasonable to select patients randomly and the two groups were comparable. In the group A, compared with the results before and after treatment, BMD:P< 0.01, E2: P< 0.05, B-ALP:P< 0.001, TRACP5b: P< 0.001. All of these are statistically significant. In the group B, compared with the results before and after treatment, BMD:P< 0.05, E2:P> 0.05, B-ALP:P< 0.05, TRACP5b:P< 0.001; compared with BMD, B-ALP between two groups after treatment, P> 0.05, that means there is no statistical significance; compared with E2, TRACP5b, P< 0.05, that is statistically significant. However, from the trend of absolute, BMD of group A is higher than that of group B and B-ALP of group A is lower than that of group B. Conclusion:1. Modified Erxian Decoction Combined with alendronate, compared with alendronate, two methods were able to effectively improve the TCM syndrome score, improve BMD and reduce the biochemical indexes of bone metabolism; but the treatment of Chinese Medicine combined with alendronate is better to relieve backache, soreness and weakness of waist and knees, warm sweating, and it can improve the level of estrogen in vivo. The absolute of various biochemical indexes after treatment were lower than that of the group B. The absolute of BMD after treatment is higher than that in the group B. Both of results illustrate that combination treatment has more advantages than alendronate.2. Invigorating the kidney and strengthening yang, nourishing marrow bone, combined with alendronate, can be useful in the precaution and therapy of PMOP, especially in improving the level of estrogen in vivo and the TCM symptoms.
