Objective To understand the change of the bone metabolic makers in osteoportic fracture,explore the application value of bone mineral density and bone metabolism in osteoportic fracture,to provide an impeccable diagnosis and treatment strategy for osteoportic fracture.Methods According to the inclusion criteria and exclusion criteria,statistical analysis60 hospitalized above 60 years osteoportic fracture patients from January 2015 to January2016 in the Ningxia People's Hospital department of orthopaedics,Inspecting the thoracolumbar vertebrae X-ray,CT and MRI of all patients,and inspecting the bone mineral density of L1-L4 and double hip.Dividing groups with the bone mineral density.The T of non-osteoporosis was greater than-2.5,the osteoporosis was less than or equal to-2.5.There were 34 patients in osteoporosis,the amount of man was 14,the woman is20,all the women were menopause,the average age is 68.7(the range of age was 60-68),the age between these two groups was no statistical significance.There were 26 patients in non-osteoporosis,the amount of man was 9,the woman is17,all the women were menopause,the average age is70(the range of age was 60-68),the age between these two groups was no statistical significance,there was comparability.Recorded the ALP,BGP,PTH,25-OH-VD and X-ran,CT,MRI of patients.According the Oswestry dysfunction index and Wong-Banker to assess the clinical symptoms,then contrasting and analyzing the bone metabolism makers andclinical symptoms,and statistical analyzing the relation of bone mineral density inspection area and age,and the difference of osteoporosis relevance ratio under different standard.And discussing the probable cause that fragility fracture patient diagnosed osteoporosis but the BMD not reach osteoporosis.Results The comparison results of age and sex in every group: the age and sex between these two groups was no statistical significance;The comparison results of bone metabolic makers in every group: the difference of PTH,BGP,25-OH-VD,ALP between osteoporosis and non-osteoporosis group were no statistically significant.The comparison results of ODI in non-osteoporosis and osteoporosis group: the average of ODI in non-osteoporosis group was57.90,in osteoporosis group was 59.67,there was no significant difference between these two groups.The comparison results of FPS-R in non-osteoporosis and osteoporosis group: the average of FPS-R in non-osteoporosis group was 7.5±0.5,in osteoporosis group was 7.9±0.7,two groups were no statistically significant.The comparison results of ages which lowest T-score of different part:the average age of lowest T-score at lumbar vertebra was 65,but at the hip the average age was 73,the age between different part was statistically significant.The osteopor0 sis relevance ratio was 75% by the diagnostic criteria of 2.0SD,and it was 50% by the 2.5SD,the different was statistically significant.In 60 patients with fragile spine fractures,the density of thoracolumbar vertebrae was reduced except for compressed vertebrae,bone trabecula were rare,bone cortex thins,appeared mullion structure wale bone trabecula,the constringent centrum was wedge-shape,biconcave and flat.Conclusions(1)Using BMD measured by DEXA is influenced by the inspection area,no universal standards for the diagnosis,country,race,the technician levels were different and it's own shortcoming,osteoporosis cannot be diagnosed accurately,it has some limitations in clinical application(2)Bone metabolism index can assess the bone quality of osteoporosis patients,it has application value in osteoporosis diagnose.(3)For agedness osteoporosis patients,the BMD unites bone mineral density,injury process and imaging manifestations canassess if the fracture caused by the osteoporosis more accurately.
