Objective:Mycoplasma pneumoniae pneumonia is increasing in the proportion of pediatric pneumonia and as we know in clinical work it belongs to the very common respiratory infectious diseases. For clinical manifestations and imaging changes of mycoplasma pneumoniae pneumonia are diversity, performance of lung signs is not very obvious, clinical serological test of mycoplasma pneumoniae antibodies needs more time, the clinical early diagnosis and treatment of mycoplasma pneumoniae pneumonia may become more difficulty. This article aims to analysis the clinically relevant indicators(immune status, infection index) about the mycoplasma pneumoniae pneumonia of the different imaging changes, to discuss its clinical significance, to make us know the characteristics of mycoplasma pneumoniae pneumonia of the different imaging changes further. Fanally this will help the doctor diagnose and cure mycoplasma pneumoniae pneumonia earlier, and avoid the process of illness and the serious effection on children’s physical and mental health.Methods:The study randomly selected 345 children with mycoplasma pneumoniae pneumonia form the second hospital of dalian medical university in three years( form January 2012 to December 2014). According to the imaging changes these children divide into two groups, one is lobar pneumonia, the other is non lobar pneumonia. The first group has 122 cases, the 223 cases is in the second group. Collect all the result of blood routine examination, blood sedimentation, c-reactive protein(CRP), calcitonin(PCT), immunoglobulin, CD4, CD8, determination of mycoplasma antibodies, chest X-ray or chest CT, clinical symptoms, clinical therapy about all children with mycoplasma pneumoniae pneumonia. And the results were retrospectively analyzed.The statistical difference between age and gender is not significant in the two groups.Results:1. The levels of neutrophils ratio in lobar pneumonia are higher than that in non lobar pneumonia mycoplasma, the levels of lymphocyte ratio in lobar pneumonia are lower than that in non lobar pneumonia mycoplasma, and the statistical difference is all significant(P<0.05). The two groups are different on mononuclear cells, acidophilic granulocyte, white blood cells, hemoglobin and platelet, but the statistical difference is not significant(P>0.05). 2. The levels of ESR, CRP and PCT are lobar pneumonia with are higher than non labar pneumonia, the difference is statistically significant(P<0.05). 3. The levels of CD4 and the ratio of CD4/CD8 in lobar pneumonia are lower than that in non labar pneumonia, but the levels of CD8 are higher than that in non labar pneumonia. The statistical difference of CD4 and CD8 between labar pneumonia and non labar pneumonia is not significant(P>0.05), but the statistical difference of the ratio of CD4/CD8 in the two groups is significant(P<0.05). 4. The levels of Ig A, Ig G, Ig E and Ig M about lobar pneumonia are higher than that about non labar pneumonia, but the statistical difference is not significant(P>0.05). 5. The frequency of glucocorticoid therapy on lobar pneumonia is higher than non lobar pneumonia, and the statistical difference in the two groups is significant(P<0.05).Conclusion:1.Because in routine blood examination the levels of neutrophils ratio of lobar pneumonia significantly increase than non lobar pneumonia, but the proportion of lymphocyte significantly decrease, it has significant value for the clinical diagnosis and treatment of different imaging mycoplasma pneumonia. 2. The levels of ESR, CRP and PCT of lobar pneumonia are higher on different extent, it tells us that pathological changes are more large, the children of mycoplasma pneumonia have the more severe inflammation. It is good for analysis of the clinical condition on mycoplasma pneumonia of different imaging changes, it has a guidingsignificance. 3. The ratio of CD4/CD8 in lobar pneumonia significantly decreases. This tells us after the mycoplasma pneumoniae infecting body immune of children isorders, pathological changes is more large, the expression is more obvious, besides it shows that the cellular immune mechanisms in mycoplasma infection plays an important role. 4. Children of lobar pneumonia have higher levels of Ig G, Ig M, Ig E and Ig A than that non lobar pneumonia, it shows that the humoral immune has changed after mycoplasma pneumoniae infecting body, there is no relationship between pathological changes and humoral immune changes. 5. The frequency of glucocorticoid therapy on lobar pneumonia is higher than non lobar pneumonia, glucocorticoid can have a good effect on treating lobar pneumonia, it has a guiding significance about therapy of lobar pneumonia.
