Objectives:Through 256-slice CT perfusion imaging,In normal liver, cirrhosis and liver cirrhosis and portal thrombosis after liver Hemodynamic changes of liver and to study the impact on the progression of cirrhosis of the liver after portal vein thrombosis..Materials and methods:256-slice CT perfusion scan of the whole liver and image post-processing were obtained in 49 individuals who included 16 control subjects,21 patients with liver cirrhosis, and 12 patients with cirrhosis and portal thrombosis。Obtain and record each group were abdominal aorta level portal vein, portal vein, liver parenchyma, spleen Substantial four ROIs (range of interest, ROI) of time-density curve (time-density curve, TDC) and peak time (time to peak, TTP). Enter the abdominal aorta as liver artery and portal vein enter the liver as the portal vein, Select another can display 4 levels of liver lobe, each lobe were obtaining blood perfusion parameters (including hepatic left lateral lobe, left inner lobe, right anterior lobe, right posterior lobe), And record the perfusion parameters, including: hepatic arterial perfusion (hepatic arterial perfusion, HAP), portal perfusion (portal vein perfusion, PVP), total hepatic blood flow (total liver perfusion, TLP), hepatic perfusion index (hepatic arterial perfusion index, HPI), Each lobe draw three ROI, A total of 12 ROI, The system automatically generates perfusion value of each liver lobe. Mean take 12 ROI as a whole liver blood perfusion parameters. Then, The hepatic perfusion parameters in the control subjects and the cirrhosis group, the Cirrhosis and portal thrombosis group. As well as cirrhosis and portal thrombosis occurred in different parts of the changes in hepatic perfusion index.Results:1. Cirrhosis group of abdominal aorta, portal vein, liver, spleen and TTP than extend the normal group, except for the TTP of aorta was no statistical difference. More than three TTP are statistically significant.2. Cirrhotic portal vein thrombosis group of abdominal aorta, portal vein, liver, spleen and TTP than extend the normal group, except for the TTP of aorta was no statistical difference. More than three TTP are statistically significant.3. Cirrhotic portal vein thrombosis group abdominal aortic TTP than without thrombosis group shorter, but not statistically TTP than significant, Portal vein, spleen and liver of TTP was prolonged than those without thrombosis, In addition to the extension of TTP spleen was no significant remaining outside both statistically significant.4. Portal System without thrombosis group compared with the control group:HAP reduction, not statistically significant. PVP and TLP reduction, HPI increase was statistically significant.5. the normal control group compare with Cirrhotic portal vein thrombosis group:HAP reduction, not statistically significant. PVP and TLP reduction, HPI increase was statistically significant.6. Cirrhotic portal system without thrombosis group compare with the cirrhotic portal vein thrombosis group:HAP, HPI increase, PVP decrease was statistically significant. TLP reduction was not statistically significant.7. Cirrhosis and portal thrombosis occurred in different parts of the perfusion of comparison:With the portal system expanding the scope of thrombotic occlusion, PVP will be gradually reduced, HAP, HPI, TLP will gradually increase, but not statistically significant.Conclusions:1.256-slice CT liver perfusion imaging is a simple method of imaging, Accurately reflect the normal liver, cirrhosis, liver cirrhosis and portal system thrombosis Hemodynamic changes trends, PVP changes contribute to the diagnosis of cirrhosis.2. Cirrhosis, portal vein, spleen and liver of TTP delayed, PVP, TLP reduced Liver, portal vein TTP delayed, PVP decreased, indicating that will increase the degree of cirrhosis and portal hypertension thrombosis.3. Thrombosis occurred in different parts of the liver among perfusion index did not change significantly.
