Syndrome Differentiation And Treatment Of Clinical Efficacy Of Primary Glomerulonephritis Hematuria Was Observed In 58 Patients

Objective:By treating with traditional chinese medicine for primary glomerulonephritis patients with bloody urine,observing the changing of TCM syndrome and urine red blood cell count before and after treatment,to confirmed the curative effect of traditional chinese medicine for primary glomerulonephritis patients with bloody urine, explore the effective methods of hematuria by traditional Chinese medicine.Methods:Collected 85 cases of primary glomerulonephritis patients with bloody urine in nephrology outpatient of hubei province hospital of Traditional Chinese medicine from February 2014 to December 2014. There are 58 cases of voluntary patients using Chinese medicine to treat, this 58 patients was collected information(including name, sex, age, diagnosis, symptoms, signs), set for the TCM treatment group. They were treated and observed for three months, follow-up every two weeks, record the symptoms integral, check kidney function、 urine analysis、blood analysis、ALT、AST、γ-GT、urine protein quantitation、 MAU/UCR determination、Ig A 、 Ig G 、 Ig M 、 ECG 、 stool routine at 0 、 12 weeks;check urine analysis 、 MAU/UCR determination at 2 、 4 、 6 、 8 、 10 weeks.Patients take different prescription according to the classification of the TCM symptom. The remaining 27 patients are reluctant to accept Chinese medicine treatment, only willing to accept oral western medicine treatment, design is the western medicine treatment group.Information collection, symptom score and blood and urine samples collecting are the same of TCM treatment group.Western medicine treatment group using only ARB/ACEI symptomatic treatment of without the syndrome differentiation and treatment of traditional Chinese medicine. Observe the changing of syndromes integral、urinary red blood cell count and 24 hour urine protein 、 urine trace albumin/uric creatinine quantitative after 3 months, the collected data compare within the group, using SPSS17.0 statistical analysis software, to discuss the curative effect of hematuria by treated with Traditional Chinese medicine.Results:1. According to the criteria of TCM syndrome curative effect, TCM treatment group total effective rate was 91.4% after treatment, western medicine treatment group was 70.4%. According to the criteria of curative effect, TCM treatment group total effective rate was 89.7% after treatment, western medicine treatment group was 66.7%. By Ridit test, with significant difference between the two groups(P<0.01), TCM treatment group is better than western medicine group.2. TCM syndrome integral comparison: By t-test, TCM treatment group and Western medicine treatment group before and after treatment showed significant difference(p<0.01). After treatment, the TCM syndrome differentiation and treatment group and Western medicine treatment group were compared with significant difference(P<0.01),the TCM syndrome differentiation and treatment group are better than western medicine treatment group.3. Comparison of urinary red blood cells: By t-test, TCM treatment group and Western medicine treatment group before and after treatment showed significant difference(p<0.01). After treatment, the TCM syndrome differentiation and treatment group and Western medicine treatment group were compared with significant difference(P<0.01),the TCM syndrome differentiation and treatment group are better than western medicine treatment group.4. 24 hour urinary protein quantitative comparison: By t test, TCM treatment group and Western medicine treatment group before and after treatment showed significant difference(p<0.01). After treatment the TCM treatment group and Western medicine treatment group showed no significant difference(p>0.05).5. Comparison of MAU/UCR: By t-test, TCM treatment group and Western medicine treatment group before and after treatment showed significant difference(p<0.01). After treatment, the TCM syndrome differentiation and treatment group and Western medicine treatment group were compared with significant difference(P<0.01),the TCM syndrome differentiation and treatment group are better than western medicine treatment group.6.The TCM syndrome types of 58 patients with Primary Glomerulonephritis and hematuria were distributed as follow. Deficiency of qi and yin with damp-heat in lower energizer had the highest rate among patients(46.6%);Secondly,25.9 percent of cases were classified as deficiency of spleen and kidney without controlling actions.Thirdly,17.2 percent of cases were distributed to deficiency of kidney yin and hyperactivity of ministerial fire and 10.3 percent to internal stasis of pathogenic heat that force blood traveling without regulariyt.Conclusion:TCM syndrome differentiation and treatment of primary glomerulonephritis hematuria curative effect significantly, can significantly improve the clinical symptoms of the patients and improve the quality of life.
