Objective :Gastric cancer is one of the most common human malignancies, and its incidence is only second to lung cancer. The number of new cases in China every year accounts for about 42% of worldwide new cases due to the dietary habits and other reasons.The cure method of gastric cancer is mainly gastrectomy. The main modes of operation include laparoscopic radical gastrectomy and conventional open gastrectomy. The conventional gastrectomy has been more mature. With the continuous development and innovation in medical technology, laparoscopic minimally invasive technology has become a new trend in recent years. The laparoscopic technique develops rapidly in our country. The curative effect of laparoscope assisted radical gastrectomy for early gastric cancer has been confirmed by the majority of clinical researchers at home and abroad. But the clinical study of advanced gastric cancer is relatively less. This clinical study was aimed to make a retrospective study on patients collected in our hospital who met the requirements. A comparative study on the perioperative curative effects between the advanced peritoneal gastrectomy and conventional radical gastrectomy was conducted to explore.Methods :A retrospective analysis was conducted on 60 cases of stage II and stage III gastric cancer patients admitted Yi Ji Shan hospital affiliated from Wannan Medcine College from June 2012 to December2013, including 30 cases of patients in laparoscopic gastrectomy group and 30 cases of traditional open gastrectomy during the same period. The operation related indexes were observed in two groups. The patients in conventional open gastrectomy were taken as the control group and patients in laparoscopic gastrectomy as the experiment group. The operation time, intraoperative bleeding, lymph node dissection, hospital stay, anus exhausting time, postoperative feeding time and postoperative complications were compared between two groups.Results:No patient died during the operation in two groups. The operation time was(270.82±43.58) min in laparoscopic gastrectomy group and(231±55.83) min in open gastrectomy group. There was statistically significant difference in two groups(P<0.05); The intraoperative bleeding was(394.34±103.87) ml in laparoscopic gastrectomy group and(381.12±90.45) ml in open gastrectomy group. There was no statistically significant difference in two groups(P>0.05); The anal exhausting time was(33.41±2.55) h in laparoscopic gastrectomy group and(70.55±6.01) h in open gastrectomy group. There was statistically significant difference in two groups(P<0.05). The anal exhausting time was earlier in laparoscopic gastrectomy group; The first feeding time was(40.43±10.08) h in laparoscopic gastrectomy group and(73.76±5.94) h in open gastrectomy group. There was statistically significant difference in two groups(P<0.05), suggesting that the gastroenteric function recovery was rapid in laparoscopic gastrectomy group; The hospital stay was(9.15±2.10) d in laparoscopic gastrectomy group and(11.26±1.38) in open gastrectomy group. The hospital stay was shorter in laparoscopic gastrectomy group and the difference was obvious(P<0.05). The lymph node dissection was(21.27±2.88) in laparoscopic gastrectomy group and(24.58±3.69) in open gastrectomy group. There was no statistically significant difference in two groups(P>0.05), suggesting that gastric cancer could achieve the same curative effect as open gastrectomy. The complications occurred on 1 case in laparoscopic gastrectomy group and 2 cases in open gastrectomy group. The complications were less. There was no statistically significant difference in two groups(P>0.05).Conclusion : The perioperative curative effect of laparoscopic gastrectomy for advanced gastric cancer is basically consistent with that of open radical gastrectomy. And laparoscopic radical gastrectomy has obvious advantages including small trauma, short hospitalization time, fast postoperative recovery and other reasons.
