| Objectives In order to reduce the incidence of thyroid disease in a healthy population,we investigated the prevalence and influencing factors of thyroid nodules in Tangshan which provided a rationale for the prevention of thyroid nodules.Methods We collected the information of 8500 people of 6 different occupation including medical staff, university teachers, public security police, bank employees, civil servants, workers, who have taken Health examination in the Medical Examination Center of Tangshan worker’s hospital from October 2013 to October 2014.Through questionnaires,physical examination and thyroid uhrasonography. According to inclusion and exclusion criteria, a total of 8500 people with the age of 23~75, including 4488 cases of male and4012 cases of female. All the data was analyzed by statistical software package SPSS 17.0.Counting information are expressed as percent, comparing use chi-squared 2 test.Multivariate Logistic regression was used to analysis of influencing factors for thyroid nodules. Test standard: P<0.05 statistical significant.Results 1 8500 occupation groups detected 3245 cases of thyroid nodules in Tangshan,the prevalence rate of thyroid nodules was 38.2%. Occupation groups were categorized according to age, compare the prevalence rate in different age, the differences were statistically significant(χ2=607.990, P<0.01), the prevalence rate of thyroid nodules was increased with the age(χ2=580.392, P<0.01). Stratified by gender, the prevalence of thyroid nodules among male and female showed an increasing trend with age(male: χ2=285.408, P<0.01, female: χ2=312.315, P<0.01). Occupation groups were categorized according to gender, the prevalence rate of female was significantly higher than male(44.8% vs. 32.3%, χ2=139.781, P<0.01). Stratified by age, the prevalence rate of female was higher than that in male in 20~, 30~, 40~, 50~, ≥60 age groups(χ2respectively=8.161, 23.958, 93.371, 22.482, 15.877, P < 0.01). Occupation groups were categorized according to different occupations, the prevalence rate was different in 6 different occupations(P<0.01). The age standardized comparison, the standardized rate was 51.0%,38.3%, 32.1%, 38.7%, 34.6%, 34.7% in medical staff, university teachers, public security police, bank employees, civil servants, factory workers. The prevalence rate in medical staff group was the highest, and then followed by, bank employees, university professors,factory workers, civil servants, police. 2 The results of single factor analysis: body mass index, regular consumption of marine products, pressure, bad mood, attitude, anxiety, the family history of thyroid disease, history of thyroid disease, blood glucose, blood pressure and the prevalence of thyroid nodules were related(P <0.05). By multi-factor Logistic regression analysis: gender, ages, occupations, body mass index, pressure, bad mood,attitude, anxiety, the family history of thyroid disease, hypertension, high blood sugar were the independent influencing factors for thyroid nodules(P<0.05).Conclusion 1 The Prevalence of thyroid nodules was high in Tangshan. the prevalence of female was higher than male and the rate was increased with the age. The prevalence rate was different in different occupations. The age standardized comparison, the prevalence rate was highest in high risk occupation, high pressure of medical staff, and then followed by, bank employees, university teachers, workers, civil servants, public security police. 2 Gender, age, occupations, body mass index, pressure, bad mood, anxiety,attitude, the family history of thyroid disease, hypertension, hyperglycemia were influencing factors for thyroid nodules. |
