Ntegrated Backscstter Technique In The Diagnosis And Staging Of Liver Fibrosis In The Application

Objectives Study of ultrasonic integrated backscatter(Integrated backscatter, IBS)technology in the diagnosis of liver fibrosis and staging of application value.Methods 2013 to December 2014 for liver diseases such as the patients of the affiliated hospital hospital of north china university of science hospital and the tangshan city infectious disease hospital and outpatient clinic111 cases of surgical treatment of patients,Among them,hepatitis B and liver biopsy of hepatic fibrosis in 39 patients,confirmed by biopsy or surgical 35 patients with liver fibrosis,confirmed by biopsy or surgical of 37 patients with liver cirrhosis.According to the pathological classification divided the patients into hepatitis, liver fibrosis and liver cirrhosis group,Other selected 42 normal healthy persons as controls.Apply the colour to exceed the AD- determination of IBS software client liver IBS each parameter values,analysis of the difference of each group and liver fibrosis stage IBS parameters,analysis of the IBS value and the correlation between liver fibrosis.Results 1.Different pathological stages of hepatic fibrosis IBS value(P<0.01),two two comparison of results show,different pathological staging of liver fibrosis differences between adjacent two groups have statistical significance(P<0.05).2.The near field and far between liver IBS value comparison:the group of liver fibrosis and early cirrhosis group liver IBS value of near far field increased compared with normal control group,there are significant differences between the two two groups(P< 0.05),hepatitis group hepatic fibrosis group near far fiend in early liver cirrhosis group showed increased IBS value,there are significant differences between the two two groups(P< 0.05);compare the value of the same group between different regions of IBS:group B group in liver IBS value decreased in the order of far field control,two two the difference was statistically significant(P<0.05),the early stage of liver fibrosis in liver cirrhosis group mid fielder IBS is higher than the value of the near field and far field and near field IBS in the two groups is higher than the value of the far field,two two the difference was statistically significant( P < 0.05).3.using ROC curve, ROC analysis of liver fibrosis and early cirrhosis by IBS of the liver near and mid far field the area under the IBS cure of the liver near and mid far field>0.7,it is suggested that the IBS value of liver parenchyma has a certain significance for the diagnosis of hepatic fibrosis.when the critical value of the liver fibrosis is near to the middle and far field is 29.71 、 35.82 、 25.16 d B.sensitivity respectively.97.2%.the IBS value of liver parenchyma can be used as the primary screening of hepatic fibrosis.the degree of specificity 68.3%.midfield can be the preferred screening indicators.The maximum specificity of area under the middle of the midfield curve is the highest,and it is suggested that it is a special indicator for diagnosis of liver fibrosis.the IBS vaiue of the essence of the middle liver can be used as the most objective index for diagnosis of hepatic fibrosis,and the IBS value of liver parenchyma is increased with the aggravation of liver fibrosis.4.Backscatter integration analysis diagram and Spearman correlation coefficient parameters of the test results show that,( r=0.889 ~0.916).IBS value and pathological staging of liver fibrosis was significantly positive correlation.IBS value of liver and liver parenchyma of two-dimensional ultrasound imaging observation, capsule, edge, vein and the total score of each observation index in good correlation with the pathological staging of liver fibrosis,among them,Liver IBS value the best correlation with pathological staging of liver fibrosis(r = 0.916).Conclusions1 With the increase of degree of liver fibrosis,liver IBS value increase,IBS technique in the diagnosis of liver fibrosis and early cirrhosis has important clinical value. 2 IBS the liver Cell change reflected out.tissue real reflected.At the same time can make a quantitative analysis of diagnosis.in the clinical application.we can use this technology for quantitative diagnosis of live fibrosis.Detection of the condition changes.provides a noninvasive method and further opportunities for clinical treatment.
