| Objective: Affects of sodium nitroprusside(SNP) and isosorbide mononitrate(5-ISMN) on plasma nitric oxide(NO) and endothelin-1(ET-1) levels in chronic heart failure(CHF) patients who were studied, so as to analyze its possible mechanisms; Compare SNP and 5-ISMN clinical efficacy for patients, and the different efficacy of SNP and 5-ISMN in those patients who were evaluated.Method: 30 patients who selected in line with NYHA Ⅲ- Ⅳ grade were randomly divided into two groups : one used SNP to treat patients and the other used 5-ISMN.Both groups were given conventional treatment.Observe and record changes in clinical symptoms. Determine the levels of NO with nitrate reductase method, before and after give the treatment to the patients. And use the same method to determine the levels of ET-1 with radioimmunoassay.Results: When used SNP to treat patients, the levels both NO( 35.30 ±7.19mmol/L) and ET-1(56.79±8.49pg/ml) were significantly lower than before treatment,which are(67.33±7.72 mmol/L)and(100.06±11.32 pg/ml). The difference has statistical significance(P <0.01); When used 5-ISMN to treat patients, the levels both NO( 44.62 ± 6.57mmol/L) and ET-1( 72.30 ± 7.25pg/ml) were significantly lower than before treatment which are(65.22±7.17mmol/L)and(97.27±9.26 pg/ml). The difference has statistical significance(P <0.01); after use SNP to treat the patients, the levels both NO and ET-1 were significantly lower than use 5-ISMN. The difference has statistical significance(P <0.01); Both SNP and 5-ISMN can improve the symptoms, and SNP is more effective. The difference has statistical significance(P <0.05);Conclusion: Both SNP and5-ISMN were able to reduce the levels of NO and ET-1. SNP is more effective. The mechanism of SNP and 5-ISMN when treat the patients may include improvement of endothelial function. Both SNP and 5-ISMN can improve the symptoms and SNP is more effective than5-ISMN. |
