Objective: Obstructive nephropathy is the most common disease in urinary system,characterised by renal interstitial fibrosis.Injury of renal tubular epithelial cells,epithelial-mesenchymal transition and deposition of extracellular matrix were its histopathological characteristics;the severe degree is related to the obstruction property,duration and complications. Establish the PUUO model in rats,laminin(LN) and matrix metalloproteinase-3(MMP-3) were choosed as monitoring indicators,to explore the degree of hydronephrosis,injury of renal tubular and atrophy of nephron during different period.To provide theoretical reference for treatment of obstructive nephropathy,time limit of follow-up and prognosis after removing obstruction.Methods: 80 SD rats were purchased from animal laboratory of Third military medical university,and randomly assigned into experimental group and control group.The experimental group were randomly assigned further into PUUO 3 days group(n=20),PUUO 5 days group(n=20),and PUUO 7 days group(n=20).Unilateral ureteral was only exposed in control group,PUUO modal were established using the ureter-psoas major muscle embedding method in experimental group.Rat were sacrificed at 3,5 and 7 days in experimental group and sacrificed at 7 days in control group after operation. Completely removal bilateral kidney and ureter,macroscopic observation renal morphologic changes,Hematoxylin-Eosin observe the renal pathological changes and count atrophy of nephron,the expression of LN and MMP-3 in renal tissues were evaluated by immunohistochemistry.Results:80 SD rats were not accidental death in the experiment.(1)Appearance observation:to compared preoperative,the size of bilateral kidneys were not different,coat and appearance were complete and tissue is ruddy luster.In PUUO 3 days group,there was no difference in size of kidneys,but the surface of kidneys were purple.In PUUO 5 daysgroup,the size of kidneys changed bigger and the vascular of kidney surface enlargement,toughness and hydronephrosis.In PUUO 7 days group,the size of kidneys further increased and translucent,capsular tension heighten,hydronephrosis further and renal cortical as thin as paper.(2)H-E observation:Glomerular and renal tubular were clearly visible,orderly arranged,size and morphology were regular,structure integrity;there was no interstitial inflammatory cell infiltration in control group.In experimental group,there were different degrees of change in renal parenchyma.In PUUO 3 days group,no obvious change in glomerular,manifold and the distal renal tubules were mild expansion,bubble degeneration in partial renal tubular epithelial cells,and inflammatory cells infiltrated in local stroma.In PUUO 5 days group,the glomerular and renal tubular collapsed,structure disorder,tubular expaned generally,epithelial cells atrophy,part of glomerular hyaline degeneration;mononuclear,lymphocyte infiltration in renal interstitial.In PUUO 7 days group,morphological structure of nephron were fuzzy,most glomerular atrophy or hyaline degeneration,no tubular structure,within the interstitial fibroblast diffuse hyperplasia,and small amount of fibroblasts,balloon tube expansion around partial glomerular residual small.(3)Immunohistochemistry observation:①LN,MMP-3 protein expression has been observed in rat renal tubular epithelial cells both experimental group and control group,but has not observed in negative control group.②Expression of LN increased,but expression of MMP-3 decreased with obstruction time.To compared with the control group,there were significant differences(P<0.05) in the PUUO 3,5,7 days group.③Atrophy of renal units extend increased obviously with obstruction time;to compared with the control group,there were significant differences(P<0.05) in the PUUO 3,5,7 days group.Related analysis has shown a negatively correlation between the proteins expression of LN and the residual nephron,and positive correlated with the atrophy of renal units;positive correlation of MMP-3 proteins and the residual nephron,but negatively correlated with atrophy of renal units.There was difference in correlation index(r)(P<0.05).Conclusion:LN,MMP-3 protein express in epithelial cells both PUUO and healthy renal tubular.Relevant expression intensity related to the duration of obstruction in obstructive renal tubular epithelial cells.Expression of LN gradually strengthen,but MMP-3 expression decreased,both played certain role in the process of renal interstitial fibrosis.Expression of LN and MMP-3 protein has related to decreased number of nephron.They were maybe regulat the apoptosis of renal units in obstructive nephropathy.
