Objective: Through the detection of HMGB1, D-lactate, IL-1, IL-10 and the apoptosis rate of lymphocyte subsets CD3+,CD4+,CD8+ in the rat with SAP to explore the role of HMGB1 in the damaging process of intestinal immunological barrier in the rat with SAP and to provide new train of thought and theory basis for the treatment of acute pancreatitis.Methods: The body weight in 200g-250 g healthy SD rats(male or female) 104, were randomly divided into five groups:sham operation group(A group,n=8),SAP model group(B group,n=24), and Qing Yi II(QY-II)-treated group(C groun=24), glutamine(Gln)- treated group(D groun=24), QY-II + Gln- treated group(E group,n=24). The sham operation group were only flipped the intestinal canal; the SAPmodel group were induced by retrograde injection of 5% sodium taurocholate into bilio-pancreatic duct; to fill the stomach with QY-II, Gln and QY-II + Gln to establish the QY-II-treated group, Gln-treated group and QY-II + Gln-treated group after the SAP rats awaken. All rats were sacrificed after SAP had been induced at 6 hours, 12 hours and 24 hours. Serumamy-HMGB1, D-lactate, IL-1, IL-10 expressions were measured by ELISA; Real-time quantitative reverse transcription polymerase chain reaction(RT-PCR) to detect the ileum-HMGB1 expression; Flow cytometry to measure Peyer ’s lymphocyte subsets CD3+,CD4+,CD8+apoptosis rate; light microscope to observe the pancreas and the ileum’s pathological changes, electron microscopic observation of intestinal mucosa pathological changes. All test results be analyzed by statistical analysis SPSS17.0 software.Results: The result show that:1. the expression of ileum-HMGB1、Serumamy-HMGB1、D-lactate、IL-1、IL-10 and the apoptosis rate of CD3+,CD4+,CD8+ lymphocyte in Peyer’s nodule in Group B were significant increased compared with Group A(P<0.01), there was positive correlation between the expression of Serumamy-HMGB1 and the severity of damage in pancreas. 2. In group C、D、E, the damage inpancreas were slight, and the expression of ileum-HMGB1,Serumamy-HMGB1,D-lactate,IL-1 and the apoptosis rate of CD3+,CD4+,CD8+ lymphocyte in Peyer’s nodule were decreased except the IL-10, when compared with Group B at 6 hours, 12 hours and 24 hours respectively(P<0.01,P<0.05). 3. Compared with group C, group D and group E, the detection index has no statistical significance(P>0.05). 4. There was no statistical significance in group C and group D(P>0.05). 5. In group B,the expression of ileum-HMGB1, Serumamy-HMGB1, D-lactate, IL-1, IL-10 and the apoptosis rate of CD3+,CD4+ lymphocyte in Peyer’s nodule at 12 h and 24 h is higher than that at 6h(P<0.01,P<0.05), the apoptosis rate of CD8 + lymphocytes in Peyer’s nodule at 6h and 12 h were increased than that at 24h(P<0.01),but there is no different between 6h and 12h(P>0.05);12h and 24 h compared that all indexes increased(P<0.01,P<0.05). 6. In group C, group D and group E, the expression of ileum-HMGB1,Serumamy-HMGB1,D-lactate,IL-1,IL-10 at 12 h and 24 h were increased than that at 6h(P<0.01,P<0.05),while the apoptosis rate of CD3+,CD4+ lymphocyte in Peyer’s nodule were decreased(P<0.01,P<0.05);the apoptosis rate of CD8 + lymphocytes in Peyer’s nodule at 6h and 12 h were increased than that at 24h(P<0.01),but there is no different between 6h and 12h(P>0.05);12h and 24 h compared that all indexes increased(P<0.01,P<0.05).Conclusion: 1.HMGB1 is involved in the course of development of SAP, and there was positive correlation between the expression level of HMGB1 and the severity of damage in pancreas.2. HMGB1 can through the adjustment of T lymphocyte subsets CD3+,CD4+,CD8+ to mediate the occurrence of IBFD when pancreatitis3. Qing Yi II can reduce intestinal immune barrier damage by reducing the Serumamy-HMGB1 expression.
