| Objective: To investigate the clinical value of detection plasma homocysteine(Hcy),cystatin C(Cys C) in patients with ischemic cerebrovascular disease(ICVD) and vascular cognitive impairment(VCI) after the illness by observing the level of Hcy and Cys C.Methods: Hcy, Cys C, FFA and another parameters were detected in 99 ICVD patients.There were 42 patients with transient cerebral ischemic attacks(TIA), 57 patients with ischemic stroke(IS) and 45 normal persons(controls) in this study, which were out of 24 patients, 26 patients with cognitive normal(CN) and 49 patients with vascular cognitive impairment(VCI) were assessed by Montreal cognitive assessment scale(Mo CA)and according the diagnostic criteria and exclusion criteria. We compared and analyzed the results with eath group and evaluated diagnosis value by drawing ROC curve.Results:① The level of Hcy and Cys C both in IS and TIA patients were significantly higher than those of normal individuals;those of IS patients were significantly higher than those of TIA patients; VCI group of Mo CA scores were significantly lower than CN group, VCI group of Hcy, Cys C levels were significantly higher than CN group;② In ICVD patients, Hcy levels correlated positively with Cys C levels(r=0.674,p=0.00);Mo CA scores correlated negatively with Hcy(r=-0.78,p=0.00)and Cys C(r=-0.62,p=0.00) levels; ③ In the IS, TIA, VCI patients, the areas under the ROC curve(AUCROC) of Hcy were 0.963,0.867,0.966,respectively,the diagnostic boundary values were 11.41 umol,10.01umol/l,14.51 umol/l;the areas under the ROC curve(AUCROC)of Cys C were 0.912, 0.808,0.768,respectively, diagnostic boundary values were 0.64mg/l,0.54mg/l,0.73 mg/l.Conclusions: There are closer relations between the level of Hcy and Cys C, both contribute to the development and growth of ischemic cerebrovascular disease, which parameters can be used to diagnosis ICVD and predict VCI after the illness,and diagnosis efficiency of Hcy is higher than that of Cys C. |
