| Objective:To analyze the correlation of cystatin C and diabetic retinopathy in type2 diabetic Patients.Methods:Record patients′genderã€ageã€diabetes duration,measure the SBPã€DBP,and measure the heightã€weight to calculate body mass index(BMI)=weight(kg)/height(m2);Serum cystatin C levels, estimated glomerular filtration rate(GFR);detect the fasting plasma glucose(FPG) 〠2h postprandial blood glucose(2h PG) 〠triglyceride(TG) ã€glycosylated hemoglobin(Hb A1c)ã€low-density lipoprotein(LDL-C)ã€creatinineã€total cholesterol(TC) 〠urea nitrogen 〠high-density lipoprotein(HDL-C) and the fasting c-peptide(FC-P) 〠postprandial 2h c-peptide(2h C-P) under the standard steamed bread meal test, all subjects were measured by immunoturbidimetry in group A(60 type 2diabetic patients with retinopathy) and group B(60 type 2 diabetic patients without retinopathy) in resting state on an empty stomach.Exam the retina by an ophthalmologist direct ophthalmoscope,fundus fluorescence angiography on occasion.Results:(1) The comparion of the general clinical data and the level of cystatin c between the two groups(table 1):The ageã€BMIã€TCã€TGã€HDL-Cã€LDL-Cã€blood urea nitrogenã€serum creatinineã€systolic blood pressureã€FC-Pã€2h C-Pã€FPGã€2h PG of the teo groups have no statistically significant difference(P>0.05); The glomerular filtration rate ã€diastolic blood pressureã€diabetes duration〠serum cystatin C level and Hb A1 c of group A was higher than those of group B(P<0.05).(2) The correlation analysis between cystatin c level with the general dinicaland biochemical index: the results show that cystatin c level of serum was positively correlated(P<0.05 或 P< 0.01) with age(r=0.377)ã€Hb A1c(r=0.176)ã€creatinine(r=0.44)ã€blood urea nitrogen(r=0.534),and has no obvious correlation with BMI 〠TG 〠TC ã€HDL-Cã€LDL-Cã€systolic blood pressureã€diastolic blood pressureã€FPGã€2h PGã€FC-Pã€2h C-Pã€diabetes duration.(3) Logistic multiple regression analyze the risk factors of diabetes retinopathy of the patients with type 2 diabetes(table 2):take whether have retinopathy as the dependent variable,take diabetes durationã€glomerular filtration rateã€diastolic blood pressureã€Hb A1cã€serum cystatin C level as independent variable,and then conduct the regression analysis.The results show that Cys C(OR=0.104,95%CI0.028-0.386,P<0.05)ã€diabetes duration(OR=0.892,95 % CI0.840-0.946,P<0.05) 〠Hb A1c(OR=0.639,95 %CI0.512-0.798,P<0.05)ã€diastolic blood pressure(OR=0.958,95%CI0.922-0.996,P<0,05)are the determinants of diabetic retinopathy in type 2 diabetic patients, namely cystatin c 〠Hb A1 c diastolic blood pressure are higher,or diabetes duration is longer,,having diabetes retinopathy is easier.Conclusion:The level of serum cystatin C may play a vital part in early preventionã€diagnosis and treatment of diabetic retinopathy.Cystatin Cã€Hb A1 c diastolic blood pressure are higher,or diabetes duration is longer, having diabetes retinopathy is easier. |
