Objective:This study aimed to evaluate the value of interleukin-6, 10(IL-6、IL-10) in early diagnosis of neonatal necrotizing enterocolitis(NEC).Analyse and compare the sensitivity and specificity of Il-6,IL-10 and c-reactive protein(CRP).Than evaluate the value of combined diagnosis in early diagnosis of NEC.Method:Selected a total of 60 infants as the research object,basing on the the modified Bell-NEC diagnostic criteria from Practice of neonatology(the fourth edition),according to whether the discharge diagnosis of NEC group,including 16 cases of stage I,20 cases of stage II or stage III, 24 cases were not NEC.Take blood sample when the NEC suspicious symptoms happened than take the after centrifugal clear liquid for determining.With double antibody sandwich ELISA method for the determination of serum IL-6 and IL-10,the scattering immune turbidimetric method for the determination of CRP.Two independent sample t test was used to P < 0.05 as the standard was statistically significant.Then compared the sensitivity,specificity,positive predictive value,negative predictive value and Youden index of each indicator.Sensitivity as the ordinate, 1-specificity as the abscissa, drow and analyzed the ROC curve to determine the optimal threshold value of diagnosis.Result:The rising of IL-6,IL-10 caused by sepsis in the results of this study may rule out.When NEC happened early, elevated CRP significantly,but the increasing of IL-6,IL-10 in stage I was not obvious while IL-6,IL- 10 increasing obviously in stage II or stage III.In the diagnosis of NEC of early stage II or stage III,the sensitivity,specificity,Youden index and area under the ROC curve of IL-6 and IL-10 are superior to CRP.The sensitivity of combined diagnosis is higher,but the specificity is low.In the diagnosis of any early stage of the NEC, the sensitivity,specificity,Youden index and the area under the ROC curve of IL-6,IL-10 and CRP are nearly close.The sensitivity of combined diagnosis is higher,but the specificity is low.Conclusion:IL-6,IL-10 for the early diagnosis of neonatal necrotizing enterocolitis has certain reference value,especially for the early diagnosis of stage II or III, combined diagnosis shows the characteristics of high sensitivity and low specificity that it can still be considered to be helpful to improve the accuracy of early diagnosis.A variety of other indicators are needed in order to improve the accuracy of early diagnosis of neonatal necrotizing enterocolitis.
